Objectives Vulvar interaepithelial neoplasia is a group of precancerous lesions which happen in vulvar epithelial cells,some may progress to invasive carcinoma.Traditional therapy such as local inunction of drugs, physical treatment and surgical treatment have relatively high recurrence rate while damaging the normal structure and function of vulvar at the same times,which bring serious psychological burdens to the paitent.So we established 129/J mice model of vulvar interaepithelial neoplasia to study the effect on vulvar interaepithelial neoplasia treated by focused ultrasound and the variance on the expression of VEGF,and the efficacy and safety of focused ultrasonic therapy.At the same time we researched and observated in the clinical for using of compound lidocaine cream topical anesthesia in focused ultrasound therapy of vulvar white lesions so as to hope relieved the sufferings of the patient from the local infiltration anesthesia in focused ultrasonic treatment.Methods To establish 129/J mice model of VINⅠ-Ⅱby injected estradiol benzoate into the abdominal subcutaneous of 40 mice for 7 weeks. After the animal model established,the VINⅠ-Ⅱ129/J mice were chosen and divided into ultrasound therapy group(8 mices) and control group(8 mices) by random.To check the pathological changes of vulvar skin and the variance on the expression of VEGF.In the research of clinical observation for using of compound lidocaine cream in focused ultrasound therapy of vulvar white lesions,90 patients which were diagnosed vulvar white lesions through the pathologic biopsy and would accept focused Ultrasound therapy in clinic were divided 3 groups at random,compound lidocaine cream+l%lidocaine 40ml(group A),compound lidocaine cream+1% lidocaine 20ml(group B),lidocaine 40ml(group C).To observe the analgesic and therapeutic effects.Results 16 VINⅠ-Ⅱ129/J mice models were established.The research results showed that 2 months after the focused ultrasonic treatment, 7 VIN mice skin returned to normal in ultrasound therapy group essentially, the cells recovered to the normal structure and polarity,the expression of VEGF decreased significantly,compared with control group the differenes had statistical significance(P<0.05).In the research of clinical observation for using of compound lidocaine cream in focused ultrasound therapy of vulvar white lesions,The VAS score was 1.60±0.86 in group A and 1.77±0.90 in group B and 8.57±1.22 in group C.The treatment times were 1246.07±421.16seconds in group A and 1606.40±503.72 seconds in group B and 1203.67±393.65 seconds in group C.The first cure rate was70%in groupA and 93.33%in group B and 63.33%in group C.Conclusion Vulvar interaepithelial neoplasia can be treated with foucused ultrasound non-invasively,effectively and safely.Compound lidocaine cream can obviously relieve the pain in local infiltration anesthesia.The analgesic effect perform well especially in mucosa of clitoris and nympha meanwhile,the use of Compound lidocaine cream can reduce the dosage of local anesthetic,decrease the edema of local tissue after injection and improve the therapeutic effect of vulvar white lesions treated by focused ultrasonic therapy.
