Objetive: To investigate the change of insulin growth factor-1 (IGF-1 )in serum in patients with essential hypertension (EH) and to reveal its effect on the level of blood pressure, left ventricular remodelling, and early changes in cardiac function.Methods: 86 patients with EH were randomly selected. All the patients were divided into three groups based on the left ventricular pattern:normal left ventricular structure (20 cases), the concentric configuration (38 cases) and the concentric hypertrophy (28 cases). At the same time 30 healthy persons were enlisted as a control group . The serum concentration of IGF-1and PIIINP was measured with ELISA and left ventricular diastolic function was measured with color Doppler echocardiography. Results:1.Pulse pressure (PP)was significantly higher in patients with EH than in control group (P<0.01). In the concentric geometry of left ventricular and concentric hypertrophy group, PP rised more obviously than normal left ventricular structure group .(P<0.05). 2.The serum concentration of IGF-1and PIIINP was significantly higher in patients with EH than in control group (P<0.05). In the concentric geometry of left ventricular and concentric hypertrophy group, the level of IGF-1and PIIINP rises more obviously than normal left ventricular structure group (P<0.05).3.The The left atrial diameter index (LADI) significantly increased in patients with EH, compared with the control group (P<0.05), but the early diastolic mitral valve flow peak velocity (MVe) and early diastolic mitral valve annulus velocity (Ea) significantly reduced (P<0.05) .4. IGF-1 in patients with EH had positive correlation with LVMI (r=0.58;P<0.05),PP (r=0.65;P<0.025),LAD(Ir=0.65;P<0.025)and PIIINP(r=0.50:P<0.05). Conclusion: 1. IGF-1 as a promoting factor may play an important role in the occurrence of hypertension, development and the process of target-organ damage. 2. IGF-1,PP and LVMI probably promote the development of hypertension through the mutual influence.3. IGF-1 may lead to early cardiac dysfunction through the promotion of myocardial fibrosis.
