BACKGROUND AND ALMS:It is reported by a lot of epidemiologic studies that Helicobacter pylori(H.pylori) is associated with some autoimmune diseases.Besides, H.pylori also possess an adhesion,BabA,which is capable of binding associated receptors of Lewis blood group antigens.There is another research applying the information biology method to find that,there exists a significant homology between human carbonic anhydraseⅡand alpha-carbonic anhydrase of H.pylori.From studies mentioned above,we can hypothesis that H.pylori infection may induce the cross immune reaction which result into autoimmune diseases,due to the molecular mimicry between H.pylori and human body.The aim of this study is trying to investigate the relationship between H.pylori infection and lupus nephritis,membrane nephrology and purpura nephritis.METHODS:Renal biopsy specimens from patients with lupus nephritis,membrane nephrology and purpura nephritis were studied using immunohistochemical labeling to clarify the aetiological significance of Helicobacter pylori antigen in these diseases.48 specimens were examined,from 27 patients with lupus nephritis,9 patients with membrane nephrology,10 patients with purpura nephritis and 2 patients with immunity associated nephritis.Immunohistochemical labeling was performed using a mixture of serum of 9 volunteers who revealed positive both in the 13C urea breath test in and confirmed by western-blot at the present study,an other mixture of serum was obtained from other 9 volunteers who revealed negative during the test in 2004 as control,and also comfirmed by western-blot.During this study,staphylococci protein A(SPA) labeled by horseradish peroxidase(HRP) was used as secondary antibody,and then all the sections were dyed by Diaminobenzidine(DAB) primarily,and redyed by Harris's haematoxylin.The sections were dehydrated by torrefied at 37℃for 2 d and then transparented with dimethylbenzene and at last mounted with neutral balsam for light microscopic observations.RESULTS:Among the 48 specimens,34/48 specimens revealed positive reaction with the anti-H.pylori positive serum mixture.The rate of positive reaction in the lupus nephritis, membrane nephrology,purpura nephritis and immunity associated nephritis respectively is 18/27,6/9,10/10 and 0/2.The reactivity was observed along the glomerular,tubular epithelium,protein cast and parietal epithelium of Bowman's capsule.The positive reactivity can be observed on a single site or a combination of the sites discussed above on one section.The SPSS software was used to analyze the data obtained from the IHC test. The statistic analysis showed that the differrrence of the positive rate between positive and negative blood-sera is significant(x2=36.318,P=0.000),and the positive rate of any two groups against positive blood-sera of the purpura nephritis,membrane nephropathy and lupus nephritis is identical(x2=13.348,P=0.001).CONCLUSION:Based on what we found from the present study,H.pylori infection may be associated with lupus nephritis,membranous nephropathy and purpura nephritis.
