Backgroud and objective:Systemic Inflammatory Response Syndrome (SIRS) is a group of clinical syndromes occurs when body loses control of Inflammatory reaction under infection or non-infection because mediators of inflammation increase greatly after over activation of inflammatory cell. SIRS is the cause of Multiple Organ Dysfunction Syndrome(MODS). CD14 is the special differentiation antigen on mononuclear macrophage surface. it is synthesized and secreted by mononuclear macrophage, and express many kinds of cytokines through mCD14 signaling pathway. stimuli to inflame- matory cell without mCD14(including neutrophilic granulocyte, endothelial cell) on LPS mainly mediated by sCD14. sCD14 is functional receptor which mediates endotoxin reaction. sCD14 can combine with LPS in cycling and elevate cell sensitivity to endotoxin, then form malignant cycling, fina- lly septic shock and multiorgan failure.integrated traditional and western medicine treatment is good in SIRS treatment, the most studied and effect drug are Rhubarb and Large Chengqi Decoction. Rhubarb can protect inte- stinal mucosa, release toxic enteroparalysis, eliminate oxygen free radicle and reduce lung injure. Large Chengqi Decoction can lower endotoxin in rat blood of SIRS, reduce content of TNF in blood and intestinal tissue and lighten mucosa injure.Method build severe acute pancreatitis SIRS rat model. including five groups, control group, SIRS group, traditional medicine group, western medicine group and integrated traditional and western medicine group. take peritoneal lavage fluid. centrifugalization in vitro and store in -80℃refrig 24h after modeling. detect concentration of sCD14 by ELISA in different groups.Result: MΦin different groups: integrated traditional and western medicine group is obviously lower than SIRS group(30.73±28.48VS 270.87±40.37,P<0.05); integrated traditional and western medicine group is obviously lower than traditional medicine group and western medicine group respectively ( 30.73±28.48 VS87.77±51.9 P < 0.05; 30.73±28.48 VS82.25±33.19 P < 0.05 ) ;there is no difference between integrated traditional and western medicine group and control group(30.73±28.48 VS30.98±23.10 P>0.05).Conclusion peritoneal macrophage sCD14 in SAPSIRS rat model, integrated traditional and western medicine group is obviously lower than SIRS group, traditional medicine group and western medicine group; there is no difference between integrated traditional and western medicine group and control group. this result implies that integrated traditional and western medicine treatment has inhibit effect on sCD14 in SAPSIRS, and is better than simple traditional medicine or western medicine treatment.
