Objective:Applying to the role that Telmisartan could anti-inflammation in the course of delaying artery hardens,considering the reduction of Central Arterial Blood Pressure as a sign of improvement to the harden artery, using the adhesion molecule that reflect the level of inflammation-Intercellular adhesion molecule-1 and vascular cell adhesion molecule-1,we could discuss the relation between inflammation and Central Arterial Blood Pressure and reveal that the reduction of Central Arterial Blood Pressure is related to the restrain of inflammation in the body in order to offer theory basis to guide the treatment to hypertension.Methods:80 hypertension patients are selected including 43 male examples and 37 female examples which are diagnosed hypertension accord with WHO/ISH hypertension diagnosis standard. The age of these patients is between 46~57 years old, average(52.12±5.6) year. The systolic blood pressure of these patients is 141.8±5.7mmHg and the diastolic blood pressure is 82.2±6.2mmHg.The 80 patients are regarded to have no disease of liver harden, diabetes, kind of rheumatism arthritis, cancer, influence at recent, operation history at recent and have no application of steroids medicine through the inspections which include blood biochemical examination, electrocardiogram, B exceed and chest flat etc. The level of low density lipoprotein (LDL) of these patients is at79-115mmol/L in the state of empty stomach. The 80 patients are divided into two groups:A group are treated with Anludipin and Amiluoli. B group are treated with Anludipin and Telmisartan. Both are followed to visit 15 months. After finishing the visiting, clinical incident such as death, myocardial infarction and brain soldier is assessed as the effect of treatment to hypertension. When the experiment begins and finishes, the elasticity of blood vessel is appraised through Ankle-Brachial Index(ABI) and pulse wave velocity(PWV) with the use of colinVP-1000. In the same time, Central Arterial Blood Pressure is also included. When the experiment begins and finishes, the vein blood of patients are gathered to detect the level of Intercellular adhesion molecule-1(ICAM-1) and vascular cell adhesion molecule-1(VCAM-1). Two groups are compared in the discrepancy of ABI and PWV including the change followed to visiting. Two groups are compared Central Arterial Blood Pressure, outer blood pressure, the level of ICAM-1/VCAM-1 including the change followed to visiting. Press two groups of Central Arterial Blood Pressure with the level of ICAM-1 /VCAM-1 to make related analysis. All of above aimed to discussing for the mechanism that the restrain of inflammation which is induced by Telmisartan could reduce Central Arterial Blood Pressure.Results:When finishing in 15 month eventually, it is 77 examples (41 male examples, female 36 examples)that follow to visit actually. In A group,38 examples is regular followed excluding 2 examples which are not willing taking the dose continuously. In B group,37 examples is regular followed excluding 1 examples which is ended because of the operation of gall bladde. The result is as follows:1.In A group's follow, ABI and PWV do not have obvious change compared with pre-following (P>0.05). In B group's follow, ABI and PWV have more obvious reduction (P<0.05).2. In A group's follow, CABP do not have obvious change compared with pre-following (P>0.05). In B group's follow, CABP have more obvious reduction compared with pre-following(P<0.05).3. In A group's follow, ICAM-1 and VCAM-1 do not have obvious change compared with pre-following (P>0.05). In B group's follow, ICAM-1 and VCAM-1 have more obvious reduction (P<0.05).4. ICAM-1's level of A and B group organize serum with CABP to contract the related analysis that get positive barrier, r value is 0.368 and 0.542 respectively; VCAM-1's level of A and B group organize serum with CABP to contract the related analysis that get positive barrier, r value is 0.502 and 0.501respectively.Conclusion:1. The effect in which Telmisartan was used to reduce Central Arterial Blood Pressure is superior to that of Co.Amiluoli.2. The Central Arterial Blood Pressure's reduce is due to restraining ICAM-1/VCAM-1's level. The Central Arterial Blood Pressure's reduce have something to do with ICAM-1/VCAM-1's level.3.The mechanism in which Telmisartan was used to reduce the Central Arterial Blood Pressure's level is that Telmisartan restrain ICAM-1/VCAM-1's level and delay artery to harden.
