There have been already 200 million people dying of tuberculosis and about 20 million people have been infected with tuberculosis in the world since Koch's found tubercle bacillus in 1882. More than 8 million new cases are diagnosed every year. China belongs to the high TB burden countries, the number of cases of tuberculosis ranks second in the world, and China is also one of the high resistance countries. About 75% of TB in China are in the age group (15 to 54 years) who can create economic value, which induced 360 million working days lost each year and caused the decline in working capacity and the lower productivity, so production structure of society has suffered a tremendous impact. Therefore, tuberculosis prevention and control should not be ignored.DOTS strategy "the short-range supervision chemotherapy under direct observation" is the global TB control strategy recommended by the WHO. It is the most effective and practical strategy which not only can reduce and prevent TB infection and TB deaths but also can control the emergence of MDR-TB. This strategy has made remarkable achievements in controlling the emergence of TB, but the results are quite from region to region. DOTS strategy in 2006 was developed into the "Stop TB Strategy" (Stop TB Strategy) by WHO, also as the basis of global TB control program in the period of 2006~2015.ObjectivesThis study will summarize tuberculosis epidemiological characteristics in the region by describing prevalent trends of the Lvyuan tuberculosis in occupation, age, gender, etc. Its target is making reasonable suggestions for the area and providing the basis for TB prevention and control work through learning the implementation and estimating the therapeutic effect of the TB control project in the region.MethodsData Source: the epidemiological data are mainly from tuberculosis quarterly statements among 2003~2007 of the Disease Control and Prevention Center in Lvyuan District; demographic data are mainly from the Statistical office and the Public Security Bureau in this area.The topic analyzes information of TB by mainly using the descriptive epidemiological method in Lvyuan District. It analyzes the dynamic change trend of tuberculosis in this area through the 2003~2007 TB epidemic situation, and further makes descriptions of the new cases registration, age and sex distribution. It also describes the occupational incidence constituent ratio of various types of tuberculosis.It observes project coverage and treatment supervisory management in this area during 2003~2007, through such as the negative conversion rate index of February and March end to reflect chemotherapy effects of various types of tuberculosis, and cohort analysis new smear-positive, relapse smear-positive and new smear-negative pulmonary tuberculosis. It reasonable assesses the implementation and therapeutic effect of the DOTS strategy in this area.Results(1)Between 2003~2007, The incidence rate of TB in Lvyuan distict showed an increasing trend. Most of TB cases were detected through clinical consultation means. The confirmed cases of TB also showed an increasing trend, and downtrend in the later. The registered new smear positive and negative TB cases had the same indentency with confirmed cases; The relapse smear positive patients fluctuated slightly; The new smear negative TB cases with free treatment showed an increasing trend year by year.(2)The gender ratio in new smear positive TB cases was 2.3:1. There were the most cases in 35~44 age-group; 25~35 and 35~45 age-group took the second place; No case was found in~14 age-group. 83.4% new smear positive TB cases were found in 15~54 age-group, as the predominant group. (3)Job-waiting group had the most propotion in new smear positive TB cases; farmers and migrant laborers took the second place. Farmers had the most propotion in new smear positive TB cases; Job-waiting group and migrant laborers took the second place.(4)The treatment rate showed an increasing trend. The rate of treatment management under direct observation reached 100% in the later stage. Between 2003-2007, The treatment rate of new smear negative TB cases showed an increasing trend; The free treatment ratereched 95%.(5)At the end of both 2 and 3 months of treatment, the sputum negative conversion rate of new and recurrent smear negative TB cases showed an increasing trend during 2003~2006;in 2007 the trendency was decreasing; the sputum negative conversion rate in 3 months behaved higher than 2 months. The cute rate of new and recurrent smear negative TB cases showed an increasing trend, otherwise failure rate had an decreasing trend.(6)No death was found in new smear negative TB cases with whole course supervision; There were some death cases in new smear positive TB cases and new smear negative TB cases with whole course management. In the begining, the mortality rate of recurrent smear negative TB cases and new smear negative TB cases with whole course management was higher than later. The mortality rate of smear negative TB cases with initial treatment keeped in a low level.Conclusions(1)Because of a large number of TB patients in this district, the epidemic situation was not optimistic. Adolescentce are prominent in the group of patients, So It had great influence on the social economic development and labor production.(2)Urban-rural low-income group had a high prevelence rate of TB in the district. It should become a focus TB prevention-control.(3)There were many important measures for the TB control, for example, improve the finding of the patient, strengthen the construction of laboratory, develop the treatment and management of tuberculosis patients. (4)During the period implementation of DOTS strategy in Lvyuan, all kinds of the chemotherapy management and the chemotherapy effect of tuberculosis patients are increasing year by year. It has achieved the project target which is achieving a high detection rate and high cure rate.
