Objective: To evaluate the curative effect of improved penile dorsal neurotomy (separate below fascia penis and selective penile dorsal neurotomy)in patients with refractory premature ejaculation.Methods: Collect the patients of refractory premature ejaculation who visited andriatry out-patient clinic from January 2004 to December 2009, If some who satisfy the standary conditions and voluntary surgery classified as experimental group, underwent improved penile dorsal neurotomy. The patients could have sexual intercourse 6 weeks after the operation and were followed up after one and a half months,three months and six months. Then assess the efficacy on the basis of intravaginal ejaculatory latency.Result: The group gain satisfactory curative effect. The intravaginal ejaculatory latency of the patients who accept the operation were significantly longer than the preoperative. Which are statistically significant (P<0.005). according to follow-up, Mean Rank were 32.80,34.35 and 34.35, the effects are stable after surgery. 13 of the 20 men reported improvement and 6 were significant. No erectile dysfunction, no ejaculation or reverse ejaculation were observed, except one patient with wound dehiscence, four patients with partial glans or penile skin numbness, which remission after three months.Conclusion: Improved penile dorsal neurotomy, Which is a safe and effective surgical for the treatment of refractory premature ejaculation, which can be considered for clinical.
