According to statistics, 75% of men in his life seen different levels of premature ejaculation. Premature ejaculation is not only to the patient's physical and mental cause great harm, but also threatens the relationship of the couple and family harmony. Therefore, the premature ejaculation has become the most important and urgent clinical topic, and urgent need us to find efficient method to solve the problem of premature ejaculation. The genetics, molecular neurobiology and endocrinology research of premature ejaculation into the causes increasing, and the understanding of the physiological mechanism and the mechanism of premature ejaculation becoming mature in recent years. Oral drugs is the main method in the clinical treatment of primary premature ejaculation, especially selective serotonin reuptake inhibitors have a definite curative effect on primary premature ejaculation, its efficient at around 63%. At present, is generally believed that the glans penis sensory nerve excitability is too high, cause ejaculation threshold low, was the cause of premature ejaculation organic cause. Through the local anesthetic drugs and penile dorsal nerve severed reduce the glans penis sensitivity can achieve the goal of treatment of premature ejaculation. Local anesthetic drugs such as compound lidocaine cream efficiently by more than 60% and the treatment of penile dorsal nerve was premature ejaculation, its actual effect is not efficient and domestic related literature reports. This study by comparing different methods for the clinical curative effect of treatment of primary premature ejaculation, specific analysis and evaluation of different treatments for the safety and efficacy of primary premature ejaculation.Methods Collected in June 2013 to November 2015 in our department of 228 patients with primary premature ejaculation, improved penile dorsal nerve severed treatment, 87 cases of patients, compound lidocaine cream to treat 61 cases of patients, Dapoxetine hydrochloride acid treatment, 80 cases of patients. Follow-up record the change of the intravaginal ejaculatory latency time(IELT) and evaluate the curative effect in the treatment of 1 month later.Results Improved the treatment of penile dorsal nerve severed in 87 patients, IELT before and after treatment were 1.13±0.50 min and 2.58±2.44 min, the difference was statistically significant(P < 0.05), 25 cases of postoperative IELT is more than 3 minutes, the effective rate was 28.7%; Compound lidocaine cream treatment group 61 examples, IELT before and after treatment were 1.14±0.55 min and 5.12±4.42 min, the difference was statistically significant(P < 0.01), IELT in 37 cases of more than 3 min after treament, the effective rate was 65.6%; On-demand oral dapoxetine hydrochloride of 80 cases, patients before and after treatment IELT were 1.11±0.52 min and 4.77±4.27 min, the difference was statistically significant(P < 0.01), IELT greater than 3 min after treatment in 49 cases, the effective rate was 61.3%.Conclusion Modified penile dorsal neurectomy is an invasive and irreversible treatments,effective rate is lower than the level of domestic related literature reports,should carefully choose the surgery; On-demand oral dapoxetine effective rate is lower than compound lidocaine cream, but it does not affect patients with erectile function and sexual pleasure.
