The Clinical Research Of Sudden Sensorneural Hearing Loss Treated By Acupuncture And Muici Therapy

[Objective]1.To observe the clinical effect and the changes of pure tone audiometry(PTA) of sudden sensorineural hearing loss (SSHL) treated by acupuncture and Muici therapy;2.To investigate the therapeutic mechanism of the therapy;3.To provide the theoretical and clinical basis for SSHL treated by acupuncture and Muici therapy.[Methods]Sixty patients with SSHL were randomly divided into two groups:the treatment group (30 cases),treated by acupuncture combined with Muici;the control group(30 cases) treated by acupuncture.Two groups were based on the same comprehensive treatments,and same with points,methods and courses.Then observe the changes of PTA before and after the treatments.The patients were classified by TCM Syndrome Types,then obverse the influence of different TCM Syndrome Types on the effects of such treatments.[Results]1.General information:There are no significant differences in sex,age, course,affected side,syndrome differentiation and symptom score in both groups(P>0.05).This study is comparable.2.TCM Syndrome Types:Based on the classification,excess patterns are more than deficiency patterns.Generaly,the exterior wind-heat patterns are most(24,52.4%);secondly are patterns of qi stagnation and blood stasis(21,35%),and then the patterns of liver fire flaming upward(7,11.7%),patterns of internal harassment of phlegm(3,5%),kidney essence insufficiency(3,5%)and dual deficiency of qi and blood(2,3.3%).The results of pure tone audiometry(PTA) show that there are three types of audiometric curve:sudden descend,slow descend,and flat.From The relationship between the TCM Syndrome Types and auditory threshold,the exterior wind-heat patterns is displayed as flat types(18,75%); qi stagnation and blood stasis is showed as flat or slow descend(18,85.7%); The specific types of kidney essence insufficiency and dual deficiency of qi and blood are sudden descend and slow descend.As for liver fire flaming upward and internal harassment of phlegm,there is no significant specifications.3.Clinical effects:The treatment group:15 cases heal completely,8 cases have marked effect,5 cases have some effect,2 cases have no effect,so the total effective rate is 93.3%.As for the Control group:6 cases heal completely,9 cases have marked effect,7 cases have some effect,8 cases have no effect,so the total effective rate is 73.3%.There is significant difference between the two groups(P<0.05),and the treatment group is superior to the control group.According to the cure rate between two groups,the exterior wind-heat patterns and qi stagnation and blood stasis have significant difference(P<0.05).The other four types have no significant difference(P>0.05).4.PTA analysis:(1)PTA types is exceedingly related to prognosis. For the treatment group,the total effective rates of PTA types are:Flat 94.1%,Slow descend 87.5%,Sudden descend 70%;For the control group,Flat 70%,Slow descend78.5%,Sudden descend 66.7%.There is no significant difference between two groups.however,from the cure rate.Flat types is 58.8% in treatment group and 21.4% in control group,There is significant difference between two groups (P<0.05).As for the sudden and slow descend types,there is no significant difference(P>0.05).(2) From the average time of effects:the time of treatment group is 7.84±4.07,and the control group is 10.48±4.46,there is significant difference between two groups(P<0.05).[Conclusions]1.Acupuncture combined with Muici therapy has definite clinical effect in treating SSHL,better than simple acupuncture.2.TCM Syndrome Types,hearing loss lever and auditory threshold are related closely,which is valued for genotyping in prognosis.3.Acupuncture combined with Muici therapy can accelerate the time of effects turning up,improve the cure rate, strengthen the confidence,so there is important value of clinic analysis.
