Objective To study clinicalpathology characteristics of the early esophageal cancer(EEC) and raise the diagnosis rate of it,to study the expression and significance of the CXCL12 and VEGF in esophageal squamous cell carcinoma(ESCC).Methods A retrospective analysis of the 122 cases of the EEC and 1215 cases of accompanying EC in the mid-late stage which proved by operation from January,2004 to December,2008 at the Department of Chest Surgery,the Fisrt Affiliated Hospital of Anhui Medical University.We recorded their detailed information using of early clinical surgery and pathology of esophageal questionnaire data of the esophageal cancer.Meanwhile,60 cases of specimens of ESCC and 60 cases of corresponding lymph node specimens and the upper cutting edge of the corresponding specimens without pathological change(as normal control specimens),which have a complete clinical material,were selected,then the expression of CXCL12 and VEGF protein in above-mentioned specimens were assesssed by immunohistochemical method.Results (78.69%,96/122)of the patients of EEC were males ,The morbidity rates in age from 50 to 69 years old was higher,(25.41%,31/122)of patients went to hospital without the esophageal symptoms(.79.51%,97/122)of the EEC localized to the middle oesophagus,squamous carcinoma accounted for ( 98.36%,120/122 ) of EEC. (61.47%,75/122)of EEC was submucosal carcinoma. The ratio of the erosive type and the block type were respectively (45%,45/100),(35%,35/100).There was no difference between them in sex ,in age and in the location of the tumor(p>0.05). 90% of the hyperaemia type of the EEC was the carcinoma in situ.The percent of the erosive type in mucous cancer was (62.96%,17/27). (44.44%,28/63) of the submucosal carcinoma was the block type. Correlation is obvious the type an size of pathdogy under gastroscopy and the depth of infliltration . The positive expression rates of CXCL12 and VEGF protein in esophageal cancer tissues were (58.33%,35/60),(63.33%,38/60), the percent of the expression of CXCL12,VEGF in the cases of corresponding lymph node specimens were (76.67%,46/60),(83.33%,50/60),respectively,were both higher than that of specimens of normal esophageal epithelium respectively(0.00%,0.00%)(p<0.05). The expression CXCL12 and VEGF in esophageal cancer were related to the depth of invasion, TNM stage and lymph node metastasis(p<0.05),There was no significant statistical difference between various patient's age groups, sex groups ,esophageal tumor site groups, and tumor length groups.The expression of CXCL12 was positively correlated with thatof VEGF in esophageal cancer(r= 0.479, p<0.05). The positive expression rates of CXCL12 and VEGF protein in esophageal cancer tissues were higher than that of corresponding lymph node (0.00%,0.00%)(p<0.05).Conclusion The erosive type and the block type were the mainly type of the EEC.An important method of early detection of the EEC was gastroscopy .Moreover,it will contribute to evaluate the depth of the tumour. we should pay attention to the patients which don't have esophageal symptoms. The expression of CXCL12 and VEGF was significantly increased in esophageal cancer tissues,and were related to the depth of invasion, TNM stage and lymph node metastasis.The expression of CXCL12 was positively correlated with that of VEGF in esophageal cancer. which suggested that CXCL12 and VEGF play a role in invasion and lymph node metastasis esophageal squamous cell carcinoma, and both have a synergistic effect.
