Clinical Analysis Of 282 Cases With Exfoliative Dermatitis

ObjectiveInvestigate the epidemiology, etiology, causative factor, clinical characteristics, treatments, curative effect, untoward effect and prognosis of exfoliative dermatitis, to understand the characteristics of exfoliative dermatitis further and guide the clinical work for the future.MethodsClinical data of the 282 hospitalized exfoliative dermatitis patients during 2000.1 to 2010.12 in the Second Hospital of Jilin University were retrospectively investigated and analysed by SPSS software including general condition, etiology, causative factor, clinical representation, laboratory investigation, treatments, curative effect, untoward effect and prognosis.Results1. Commonly condition:Morbidity was high in elder people,the mean age was 50.7±18.00 years old.Men outnumbered women in a proportion of 3.15:1.2. Etiology:80.14% patients were caused by exacerbation of the previous dermatosis, among them patients with psoriasis accounted for 69.15%.3. Causative factor:64% patients of exfoliative dermatitis caused by exacerbation of the previous dermatosis were precipitated by using medicine incorrectly,among them traditional Chinese medicine or folk prescrition account for 27%, corticosteroids 19%,topical irritants 18%, upper respiratory infections 17%. 4. Clinical representation:57.09% patients had a fever,46.10% appeard edema on legs or other position,14.54% accompanied lymph node enlargement.5. Laboratory investigation:Hypoalbuminemia was present in 62.06% patients,WBC was increased in 38.30% patients.the patients had different etiology present different laboratory investigation.6. Treatments and Curative effect:The rate of healing was 19.15%,availability was 48.94%,23.40% patients had improved equivalently,only 7.10% patients did not have obvious improvement, 1.42% patients died.The major reasons of death was the failure of organs,infection and electrolytic disorder.The patients with different causes were treated by different methods. Acitretin combined with liquorice and immuo inhibitor comported great effects on treating the patients with exfoliative dermatitis who caused by psoriasis.The curative effect was same among the patients who had different etiology, was different between young and middle-age people and elder people, elder people were poor in the curative effect than young and middle-age people.7. Untoward effect and Prognosis:Untoward effects presented during treatments were mucosa dry,damnification of liver,oral candidiasis, uncomfortable of gastrointestinal tract,et al.29 cases relapsed, someone relaped repeatedly,the most one was hospitalized 4 times.Conclusion1. Morbidity was high in elder people,men were more than women.2. Most patients were caused by other dermatosis,psoriasis is the most one among them,and the major trigger was using medicine incorrectly. 3. Exfoliative dermatitis accompany with systemic representation.4. The laboratory investigations were different between the patients with exfoliative dermatitis caused by psoriasis and non-psoriasis patients in some items.5. Acitretin combined with liquorice and immuo inhibitor comported great effects on treating the patients with exfoliative dermatitis who caused by psoriasis.6. The curative effect was same among the patients who had different causes.7. Old people were poor in the curative effect than young and middle-age people.
