Intraoperation autotransfusion means that regarding bodycavity hematocele or'operative region oozing of blood as bloody origin, after being dealed by anticoagulated,filtrated,resorpted, then refused to patients. Because it overcome many defects from foreign body transfusion.Thus it is extensively used in surgery fields. Although with so many advantages, it still do harm to erythrocyte more or less, which has been verified by many reports. It has been reported that medicine had protective function on the immune function for example TMP. We used Ligustrazine injection fluid on patients who undergo autotransfusion and compared with the control group. Then we investigate the changes of erythrocyte and TMP on how to effect these immune parameters, and so as to provide a good blood conservation measures. It could provide information for clinical study.ObjectiveTo investigate the change in immune globulin, cytokines, SOD and MDA after autologous blood transfusion, on the basis of which to evaluate the protective effect on immunity by ligustrazine.MethodsPatients of ASAⅠandⅡaged 20-60 yr scheduled for elective spinal surgery were randomly assigned to ligustrazine autotransfusion group (study group) and autotransfusion group (control group)(n=30). The patients without any blood and endocrine system diseases and excluding the cancer patients. Evaluated bleeding between 500ml and 1000ml was selected for the investigation before surgery. In study group, ligustrazine with the dose of 2mg/kg had been injected intravenously to the patients 30min before their blood was collected. And ligustrazine was put into the washing saline and heparinized saline solution up to the concentration of 0.005%. Shed blood of both groups were collected and heparinized with the same autotransfusion device (Medtronic autolog). The red cells were filtrated, separated and washed before reinfusion to the patients.recording the amount of blood loss and transfusion. Blood sample were taken before operation (To, baseline), 1h, 1d and 5d after operation(T1, T2, T3) for determination of plasma IgG,IgM,IL-2,IL-10,IFN-γ,MDA concentration and SOD activity. As the negative control, no ligustrazine was put into the washing saline and heparinized saline, at the same time, no ligustrazine was injected to the patients.Results1 Humoral immunity1.1 IgG concentrationThe plasma IgG concentrations of control group were significantly lower at T1-3 as compared to the baseline values at To (P<0.01);The plasma IgG concentrations of study group were lower at T1-2, but at T3 the concentrations had restored to the preoperative level. At T2-3 there were statistically significant between the two groups (P<0.05 or P<0.01).1.2 IgM concentrationThe plasma IgM concentrations of study control were significantly lower at T1-3 as compared to the baseline values at To (P<0.01), The plasma IgM concentrations of study were lower at T1-3as compared to the baseline values at To, but there was no statistical significance.There were statistically significant between the two groups (P<0.01).2 Cellular immunity2.1 IL-2 concentrationThe plasma IL-2 concentration were significantly decreased at T1 as compared to the baseline values at To (P<0.01); at T2 The IL-2 concentration was decreased (P<0.05). at T3 the experimental group was decreased (P<0.05), the control group was also decreased (P<0.01), There were statistically significant between the two groups (P<0.05orP<0.01).2.2 IL-10 concentrationThe plasma IL-10 concentrations were significantly high at T1-2 as compared to the baseline values at To (P<0.01), at T3 the experimental group was decreased (P<0.05), the control group was high(P<0.05), There were statistically significant between the two groups (P<0.05) at T1,T3.2.3 IFN-γconcentration The plasma IFN-γconcentrations were significantly low at T1-3 as compared to the baseline values at To (P<0.05), but the control group was not statistically significant.There were statistically significant between the two groups (P<0.01).3 Nonspecific immunity3.1 SOD acticityThe SOD acticity was significantly decreased at T1-2 as compared to the baseline values at To (P<0.01); at T3 the experimental groups were relatively higher than the preoperation (P<0.05), the control groups were lower than the preoperation (P<0.01), There were statistically significant between the two groups (P<0.01).3.2 MDA concentrationThe plasma MDA concentrations were significantly high at T1-2 as compared to the baseline values at To (P<0.01); at T3 in the experimental group MDA concentrations recovered to the preoperative level (P>0.05). the control groups were higher than the preoperation (P<0.01), There were statistically significant between the two groups (P<0.01).conclusionBoth groups could cause a certain degree depression on immunity during intraoperative blood salvage. the study group had less inhibitory impact and rapid recovery on cytoimmunity compared with the control group. The application of ligustrazine in autotransfusion could significantly bring about protective effects on cytoimmunity.
