Clinical Study On Jiawei Qihuangyin Therapy On RBP In Early Diabetic Nephropathy

BackgroundDiabetic Nephropathy is the most important microvascular complications of diabetes, and it is an important prognostic factor in the decision. Once diabetic patients developed into irreversible and persistent proteinuria, then he ultimately will be developed into the end stage of renal failure. Based on annual growth of Diabetic Nephropathy, the reversibility of early interventional treatment and the deficiency of the treatment of western medicine currently, It is particularly important and urgent to research and changing an effective drug to prevente and control the development of early Diabetic Nephropathy. With the constantly deepening of its studies on the Diabetic Nephropathy, it is found that the treatment of traditional Chinese medicine not only can reduce the level of blood sugar and proteinuria, recover and protect the kidneys from injury, delay the loss of renal function and the progression of renal dysfunction induced, but also be a little adverse reactive,cheap and acceptant modus. Therefore, by observing the clinical symptoms,RBP,UAER of therapy of Jiawei Qihuangyin in early Diabetic Nephropathy, the purpose of this study is to changing a cheap and effective traditional Chinese treatment.ObjectiveTo observe the clinical effects of therapy of Jiawei Qihuangyin in early Diabetic Nephropathy with Qi-Yin deficiency and the changing level of the retinal binding protein before and after treatment.MethodsA random control study design was used in this study. Sixty cases from early Diabetic Nephropathy (DN) with Qi-Yin deficiency were randomly divided into two groups:treatment group and control group. Patients in both groups were received basic treatments including controlling diet, lowing blood glucose and oralling taking fosinopril, 10mg a day lasting eight weeks. Additionally, patients in the treatment group orally took Chinese soup, twice a day lasting eight weeks. The changes of clinical effect, retinal binding protein (RBP),urinary albumin excretion rate (UAER), fasting blood glucose (FBG), glycated hemoglobin(HbAlc), blood urea nitrogen (BUN), serum creatinine (Scr), alanine aminotransferase (ALT) and integral of clinical symptoms were observed before and after treatment.Results1,Before treatment, there is no significant difference between age, sex, RBP, UAER, FBG, HbAl,Scr, BUN, ALT, the two group is comparable.2,After treatment, the therapeutic effect in the treatment group was markedly effective in 16 cases, effective in 10 cases, invalid in 2 cases, with total response rate of 86.67%. While in the control group, markedly effective in 11 cases, effective in 7 cases, invalid in 12 cases, with total response rate of 70%. There is significant difference on the total response rate. The treatment group was better than the cotrol group on improving the clinical symptoms.3,There was significant difference in total response rate in each group. After treatment, RBP, UAER, FBG and HbAlc were decreased in each group. But before and after treatment those was very significantly decreased, and also there was significantly different as compared with control group.4,The results of clinical syndrome after treatment were improved apparently as compared with those before treatment in each group, indicating significantly statistical difference in each group comparison. Therapies of the treatment group were superior to those of the control group in improving main clinical symptoms.5,After treatment, BUN, Scr and ALT were no significant difference in each group but before and after treatment those was no significantly different, and also no significantly different as compared with control group. There is no any adverse reaction in the experiment. ConclusionJiawei Qihuangyin has functions of reducing RBP, improving main clinical symptoms, reducing the damage of renal tubules and delaying kidney function failure of early Diabetic Nephropathy (DN) with Qi-Yin deficiency. And it can reduce urinary protein, remissing high glomerular filtration, recovering and protecting the kidneys from injury, delaying the loss of renal function and the progression of renal dysfunction induced. It also can effectively reduce the level of FBG and HbAlc, improve glucose metabolism, reduce high blood sugar by unglycoslated protein, reduce high gomerular filtration and decrease in the changing of the structure and function of kidneys. It is a safe and effective method to treat early Diabetic Nephropathy (DN).In short, Jiawei Qihuangyin is a safe and effective prescription to treat early Diabetic Nephropathy (DN) with Qi-Yin deficiency. And integrated traditional Chinese and western medical treatment was better than single western medical treatment, especially in improving clinical symptoms.
