Background:Lower deep venous thrombosis (LDVT) is a common peripheral vascular disease. The incidence of peripheral vascular disease is about 40%.In the past two decades, the morbidity of patients with DVT have been increasing gradually, and there are about three millions patients in our country,which making it now a most common disease to harm people's health. If the patient is not diagnosed and treated at the acute stage of LDVT, they may accompany pulmonary embolism (PE) because of the thrombus ablation. It is correlated reported that one millions patients should die of PE. Even though some patients survived, they have long-term complaints, affecting their normal living and working. Thereby, it is very important for us to study the early diagnosis and treatment and evaluate curative effect of LDVT in our clinic work.Objective:To study the variation of platelet surface P-selectin selecting (CD62P), lysosomal granule glycoprotein (CD63) and plasma D-dimer (D-D) in patients with DVT in lower before treatment for thrombolytic therapy, anticoagulation and anti-platelet aggregation activity and after treatment .Cases and Methods:In the department of vascular surgery in the first affiliated hospital of Anhui medical university in Hefei 31 patients with confirmed DVT of the lower limbs and 28 normal without vascular disease for outpatient in minor surgery were included. 2ml whole blood, anticoagulant by natrium citricum, were obtained at the time the patients in hospital, defined by before treatment(BT).After 72 hours of medical treatment for thrombolytic therapy and anticoagulation and anti-platelet aggregation activity, a second specimen were obtained, and after 7 days of medical treatment ,and a third were obtained defined by after-treatment(AT).CD62P and CD63 expressed on platelets surface were measured by flow cytometry, and plasma D-D was measured by LIATEST. The changes of these parameters were compared before and after treatment. To explore the relationship between them and the perimeter of both lower limbs .Result:1.CD62P and CD63 positive expressing rate in LDVT group before therapy was obviously higher than the control group(P<0.01).after 3 days of treatment for thrombolytic therapy ,anticoagulation and anti-platelet aggregation activity, the levels of CD62p were not significant difference compared with the levels before therapy(P>0.05),the levels of CD62P in those signs were apparently decreased(P<0.01),and the levels of CD62P were significant difference compared with the levels before therapy. and after 7 days of treatment the levels of CD62p and CD63 were significant difference compared with the levels before therapy(P<0.01), CD62P and CD63 positive expressing rate was higher than the control group.2. D-D content in LDVT group before therapy was obviously higher than the control group (P<0.01). D-D content was obviously higher after 3 days of treatment, and D-D content was significant difference compared with the levels before therapy(P<0.01). After 7 days of treatment ,D-D decreased to the levels before 3 days of treatment, and D-D was similar level compared with the levels before therapy(P>0.05),but D-D content was higher than the control group.3. In the course of treatment, The drop of the perimeter both lower limbs decreased continuously. before therapy the perimeter difference between the suffered limb and the healthy one were significant difference compared with the levels after 3 days of treatment and 7 days of treatment, moreover, after 7 days of treatment the perimeter difference were significant difference compared with the levels after 3 days of treatment(P<0.01).4. The change of CD62p and CD63levels was accorded with the drop of the perimeter difference between the suffered limb and the healthy one. Conclusion:1. The CD62P,CD63,D-D levels are closely correlated to the LDVT.2. CD62P,CD63 and D-D involved in formation of LDVT ,and detecting the continuing changes of CD62P,CD63 in LDVT patients'blood seem to be a important and useful indicator in the early diagnosing LDVT and evaluating curative effect for treating LDVT. They will also provide a new way for therapy of LDVT, And they were seen to be the use of dose reference at the base of these indicators.3. CD62P and CD63 positive expressing rate and D-D content in LDVT group before 7 days of treatment was obviously higher than the control group. LDVT patients discharged from hospital are still the high risk group of thrombosis, so they should be followed up regularly.
