Objective To analyze the risk factors and relation of retinopathy and urinary albuminexcretion in hospitalized patients with type2diabetes.Methods To collect complete data of225hospitalized patients with type2diabetesin our hospital endocrinology department from July2009to September2011.According to the urinary albumin/urinary creatinine rate(ACR), A1l the patientswere divided into normal albuminuria group (NA) and diabetic albuminuriagroup(DN), which was divided into microalbuminuria group(MAU) andmacroalbuminuria group (CAU). All the patients were examined by Digital retinalphotographs using a TRC-NW300(Topcon, Tokyo,Japan) non-mydriaticcamera,then them were divided into normal retina group(NR)and diabetic retinopathygroup (DR),the latter consisted of background diabetic retinopathy group (BDR) andproliferative diabetic retinopathy group (PDR) according to fundusphotography.Blood lipid(HDL,LDL,TG and TC),HbA1c and thyroid function(free T3,free T4and TSH) also were tested in all the patients, blood pressure and course ofdisease were recorded.Results1The percentages of DR and DN of this cohort were37.3%and36.9%.Thepercentage of DR and DN had no statistical difference between male andfemale((37.1%VS37.6%,38.8%VS34.9%,P>0.05). 2The percentage of DR was31.7%,40.8%and55.9%in NA, MAU and CAUgroup, respectively. There was significant difference among different albuminuriagroup (P<0.05).3The percentage of albuminuria was31.2%,39.4%and84.6%in NR,BDR andPDR group respectively, There was significant difference among different DR group(P<0.01).4Albuminuria,diabetes duration, hemoglobin A1C,and TSH were independentlyassociated with DR by Logistic regression analysis,albuminuria (OR:1.557;95%CI:1.021,2.375;P<0.05),diabetes duration (OR:1.056;95%CI:1.008,1.107;P<0.05),TSH(OR:1.185;95%CI:1.023,1.373;P<0.05),HBA1C(OR:1.157;95%CI:1.018,1.315;P<0.05).Meanwhile,diabetes duration, systolic pressure,totalcholesterol, and triglyceride were all independently associated with albuminurai,diabetes duration (OR:1.093;95%CI:1.037,1.152;P<0.05)、SBP(OR:1.047;95%CI:1.027,1.068;P <0.01)、TC(OR:1.505;95CI:1.025,2.211;P<0.05)、TG(OR:1.442;95CI:1.050,1.980;P<0.05).ConclusionsAlbuminuria, diabetes duration, hemoglobin A1C and TSH were dependent riskfactors of DR in type2diabetic patients; Diabetes duration, systolic pressure,totalcholesterol and triglyceride are the major risk factors of albuminuria of type2diabetics;Diabetic retinopathy is closely correlated with urinary albuminuria inpatients with type2diabetes....
