Mental health is one of the significant factor to measure the individual health level. Teen-agers' mental health is related to our country population quality and population health, so carrying on mental health education for teen-agers seems indispensable. And mental health education certainly relates to student's evaluation of mental health level. As a result, selecting the fit evaluation form and compiling the fit evaluation tool are the key step to resolve the problem about promotion of teen-agers' mental health. In this field there have been important developments in abroad especially on models and instruments. Compared with those researches our study appears quite weak. The aim of this study is to compile a mental health questionnaire for junior middle school students.Based on the literature research, scholastic practice, some common mental blocks of junior middle school students, an open-ended questionnaire and interviews with some teachers and students, a mental health questionnaire that adapt to Chinese junior middle school students was developed.The main conclusions are:(l)The study confirms the nine factors of mental health questionnaire, namely study habit, exam anxiety, loneliness, affiliate with opposite sex, self-government, communicate with others, the ability to bear the defeat and depend on others, inferiority complex, self-perfect.(2)The homogenous reliability of questionnaire is comparatively high and can reach the demand of questionnaire compilation. But the homogeneous reliability of the three subscale as self-government and communicate with others is under 0.60 and need to be improved further.(3)The study test the validity of questionnaire from content validity, construct validity and criterion-related validity.
