Clinical And Experimental Study On Treatment Of Diabetic Retinopathy By Qingre Huayu Decoction

ObjectiveTo observe Qingrehuayu recipe on treating non-proliferative diabetic retin opathy (NPDR) patient’s visual acuity, fundus lesions, macular retinal thicknes s, aqueous humor levels of VEGF and TCM symptom, looking for the objective evidence of Qingrehuayu recipe treat the of stasis thermal collateral type NPD R and improve related symptoms, explaining it’s therapeutic mechanism.MethodSelect 2015.1-2015.12 forty-four patients (two eyes surgery patients enrol led only one eye) in Beijing University of Chinese Medicine Dongzhimen Hos pital’s Eye Clinic who underwent cataract surgery and combined with type 2 d iabetes,diagnosed as NPDR by the optical coherence tomography (OCT) and op hthalmoscopy and (or) fundus fluorescein angiography (FFA), and met the sta sis thermal collateral type standards. They were randomly divided into two gro ups,22 cases were in the observation group and the others were in the control group.Before surgery, observation group was qiven Qingrehuayu recipe, the c ontrol group was Dobesilate calcium capsules, treatment for four weeks. Sched uled surgery 3 days after stopping the drug.when was in group and after 3 days of surgery, two groups of patients underwent examination of visual acuity, OC T, ophthalmoscopy and fundus photography, fill in the scale of clinical sympto m of TCM, surgery were taken in the eyes of the aqueous humor 80-120μL. R ecorded and compared before with after treatment and between groups and wit hin groups of visual acuity, fundus lesions, macular retinal thickness and TCM symptom scores and ocular VEGF levels.Results(1) The curative effect judgment Diabetic Retinopathy (DR):After treatment, t he observation group’s total effective rate was 59%. In the control group, the t otal effective rate was 14%. Two groups of total efficiency had significant diff erence (P<0.01). Observed group was better than the control group significan tly.(2) Vision:The effective rate of the observation group was 91%, the control gr oup was 100%. The effective rates of the two groups had no significant differe nce (P>0.05).(3) The macular retinal thickness:In the observation group, after the treatment, the macular retinal thickness was significantly lower than before, and had stat istical difference (P<0.05). The control group had no statistical difference be fore and after the treatment.(4) VEGF levels in aqueous humor:after treatment, the two groups were comp ared within groups, VEGF levels were significantly different (P<0.01), and th e two groups showed a gradual increase in VEGF level with the development o f DR. Between groups, the levels of DRⅡ-Ⅲ in the two groups were statistical ly significant (P<0.05), and the observation group was lower than the control group.(5) Clinical symptom of TCM:The total effective rate of the observation group was 83%, the control group was 5%. Two groups had significant difference (P <0.01).Conclusions(1) Qingrehuayu recipe on the treatment of NPDR can improve the fundus dise ases of patients such as micro hemangioma, hard exudates, cotton wool spots. By reducing the macular retinal thickness, alleviate macular edema. It can prev ent and treat the retinal angiogenesis and other critical situation that result of 1 evel of VEGF excessive increase, by suppresses aqueous humor VEGF levels i n a certain range. The therapeutic effect of this disease is better than western medicine Dobesilate calcium capsules.It can delay the process of the non proli feration period to the proliferation effectively, to play the advantages of traditi onal Chinese medicine in the treatment of DR, to provide a positive reference f or clinical treatment in the future.(2) Qingrehuayu recipe has obvious curative effect on the treatment of NPDR patients’the whole body unwell symptom of stasis thermal collateral type. Th ese patients had blurred vision, eye dryness, numbness, tingling, constipation, fanre, dry mouth and throat and so on, Qingrehuayu recipe according to the the ory of treatment of TCM, effective symptomatic treatment to alleviate the suff ering of patients. This laid the theoretical foundation of Xiaoke eye disease sy ndromes of traditional Chinese medicine and traditional Chinese medicine trea tment Xiaoke eye disease.
