Purpose:To observe the clinical curative effect of unstable angina pectoris of coronary heart disease with phlegm and blood stasis syndrome and explore the effect and mechanism of vascular cell adhesion molecule-1(vascular cell adhesion molecule 1, VCAM-1) treated by eliminating phlegm and dispersing blood stasis method combined with enhanced external counter pulsation(EECP).Material and method:Selecting 120 patients of unstable angina pectoris of coronary heart disease with phlegm and blood stasis syndrome, randomly divide them into experimental group and control group, 60 cases in each group. The experimental group used Wenban decoction and EECP combined with conventional western medicine treatment, the control group using Wenban decoction combined with conventional western medicine treatment. Collecting index in the two groups of subjects, such as the baseline data the angina symptoms before and after the treatment, TCM syndromes, the end point, vascular cell adhesion molecule-1(VCAM-1) and the safety evaluation, etc.Results:1.baseline: baseline of the two groups are equilibrium and comparable.2.Curative effect of angina symptoms: comparison between the two groups after treatment, the experimental group is better than the control group(P=0.000). The symptoms of angina pectoris integral: comparison between the two groups after treatment, the experimental group is better than the control group(P=0.003).3.Effect of TCM syndromes: comparison of significant efficiency between the two groups after treatment, the experimental group is better than the control group(P=0.000). Syndrome integration: comparison between the two groups after treatment, the experimental group is better than the control group(P=0.000).4. Endpoint events: The test group, 1 case of acute myocardial infarction, 1 case of readmission. The control group, 2 cases of acute myocardial infarction, 2 cases of stroke, 5 cases of readmission.5.VCAM-1 level: Both the test group and the control group are decreased after the treatment(P=0.000). comparison between the two groups after treatment, the experimental group is better than the control group(P=0.000).Conclusion:1. Eliminating phlegm and dispersing blood stasis method combined with enhanced external counter pulsation(EECP) may improve the angina symptoms and TCM syndromes, degrade the VCAM-1 expression level in patients with unstable angina pectoris of coronary heart disease, and have certain intervention effect on long-term prognosis.2.Eliminating phlegm and dispersing blood stasis method combined with enhanced external counter pulsation(EECP) have exact curative effect and good safety, which provide the basis for its clinical effectiveness and safety.
