Objectives To know the current status and influential factors of occupational stress and comprehensive work ability of production laborer in a steel enterprise to provide evidence for occupational strain and declined work ability prevention and intervention.Methods Based on stratified cluster sampling, 914 production laborers who were on duty from iron smelting processing unit, steel smelting and rolling processing unit and power provision unit in a steel enterprise were investigated anonymously by a self-administered questionnaire. The content of the questionnaire includes questions regarding demographic, lifestyle and occupational characteristics of the object, chinese version of Occupational Stress Inventory - Revised Edition (OSI-R) and Comprehensive Work Ability Index (CWAI). Tookα=0.05 as statistic significance.Results For occupational stress: The score of scales of the occupational role questionnaire(ORQ), role overload(RO), role insufficiency(RI), role ambiguity(RA), personal strain questionnaire(PSQ), vocational strain(VS), interpersonal strain(IS), personal resource questionnaire(PRQ), social support(SS), rational/cognitive(RC) were lower and the score of scales of role boundary(RB), physical environment(PE), psychological strain(PSY), recreation(RE), self-care(SC) were higher when compared with the production laborer group norm. As compared with the occupational stress norm of Southwest China group, the score of scales of RO, RI, RA, IS, PRQ, SS, RC were lower, and the score of scales of RB, PE, PSQ, VS, PSY, PHS, RE, SC were higher. Based on the score of OSI-R scale of production laborer group norm, the 97.9% of investigated object were classified as normal level or relative absence of stress on ORQ, 76.2% were classified as normal level or relative absence of strain on PSQ, 80.7% were classified as average or strong levels of coping resources on PRQ. The risk of stress on occupational role was higher in group characterized as male, work more than 45h a week, iron smelting processing unit and steel smelting unit.The risk of lack in personal resource was higher in group characterized as age more than 50y, family monthly income per capita less than 1000 yuan, night shift. The risk of personal strain was higher in group characterized as residence in countryside, college education and above, family monthly income per capita more than 3000 yuan, alcohol drinking, current-job service time less than 5y, stress on occupational role.For comprehensive work ability: The score of CWAI in investigated objects was 43.5±6.0. The risk of declined comprehensive work ability was higher in group characterized as family monthly income per capita less than 1000 or more than 3000 yuan, iron smelting processing unit, lack in personal resource, personal strain. The risk of declined physiological function was higher in group characterized as family monthly income per capita less than 1000 or more than 3000 yuan, iron smelting processing unit and power provision unit, lack in personal resource, personal strain. The risk of declined psychological state was higher in group characterized as stress on occupational role, lack in personal resource, personal strain. The risk of declined social function was higher in group characterized as worked in iron smelting processing unit and power provision unit, lack in personal resource, personal strain. Even when controlled for the demographic, lifestyle and occupational characteristics, there were still strong associations between stress on occupational role, personal strain, lack in personal resource and declined comprehensive work ability.Conclusions Some intervention programs on occupational stress should be preferentially devised and implemented by the investigated steel enterprise were physical environment improvement, job delivering and clearifying, social support resources expanding, professional training and health education, ergonomics and personal protection from hazardous agents in the workplace for the production laborer. The comprehensive work ability improvement should include many facets from physiological, psychological functions to social adaptability that could potentially influence one's comprehensive ability. Emphasis on frail group when intervene with occupational stress and comprehensive work ability. Since the strong association between the occupational stress and comprehensive work ability and the other factors, the improvement of occupational stress and comprehensive work ability should execute simultaneously to get the more effective intervention outcome.
