ObjectivesTo explore the diagnostic value of GP73 protein in gene detection in the patient of primary hepatic carcinoma,to discuss the joint role of serum GP73 and AFP in the diagnosis of PHC ,to provide a new sensitivity and specificity,simple and quick way in the census of high-risk population .MethodsUsing enzyme-linked immunosorbent method to detect the serum level of GP73 and AFP in 73 cases of PHC ,13 cases of hepatic cirrhosis , 32 cases of hepatitis and 62 cases of health people .Using SYBR Green real time fluorescence quantitative polymerase chain reaction method to detect the relative value of GP73 mRNA in the peripheral blood cells of each group.8 cases of normal liver tissues and 8 cases of PHC tissues were detected at the same time to compare the relative express difference.ResultsThe results of Kruskal-Wallis test showed that the serum level of GP73 and AFP had significant difference between four groups(H value were 89.6,52.0,P<0.01),and the whole blood GP73 mRNA had no significant difference(H value was 4.33,P=0.228),with the level of 13.65(11.89~13.91),14.18(13.40~14.87),13.07 (12.61~13.58),13.46(12.15~13.83)independently.The results of Mann-Whitney test showed that the serum level of GP73 had significant difference between PHC group [166.7(162.7~231.8)μg/L] and liver cirrhosis [57.3(46.6~113.6)μg/L], hepatitis [23.6(26.2~54.5)μg/L],health group[20.9(20.8~29.4)μg/L] (U value were 245.5,296.5,348.5,P<0.01);The AFP level of the four groups were 16.7(258.5~503.23)μg/L,3.8(1.3~14.5)μg/L,2.3(2.4~7.8)μg/L,2.8(2.2~5.7)μg/L, and it also showed significant difference (U value were 246,419,790,P<0.01). The GP73 mRNA expression of PHC liver tissue(12.64) was significant higher than normal liver tissue (1.00). The critical value was determined through the ROC curves ,for GP73 123.15μg/L,AFP 10.63μg/L. Under the critical value the sensitivity of GP73 and AFP were 65.8%,53.4% ,and the specificity of GP73 and AFP were 95.3%,92.5% respectively.Combining detection could increase the sensitivity up to 79.5% ,and maintain the specificity in a high level 90.7%.ConclusionsAs a new diagnositic marker of primary hepatic carcinoma ,GP73 protein had the very good sensitivity and specificity ,and for diagnosis of PHC,GP73 was superior to AFP.The GP73 mRNA detection could not be a diagnosis of PHC in the whole blood sample ,but be a good diagnosis of PHC in the liver tissue sample . The combine detection of GP73 and AFP could improve PHC diagnosis ,and provide an effective approcach to census the PHC high-risk screening. The tests comfirmed that GP73 was most likely to be the best serum marker of primary hepatic carcinoma.
