Objective To explore the significance of combined detection of alpha-fetoprotein(AFP)?Glypican 3(GPC-3)and Golgi protein 73(GP73)in diagnosis of primary hepatic carcinoma,and to explore the significance of Glypican 3(GPC-3)and Golgi protein 73(GP73)in diagnosis of AFP negative primary hepatic carcinoma.Methods A total of 140 patients admitted to hospital from March 2015 to October 2016 were selected,50 cases of PHC,30 cases of Cirrhosis,30 cases of Benign hepatic lesions and 30 cases of healthy control were selected.Serum AFP?GPC-3 and GP73 were measured respectively.The detection results were compared among 4 groups.Results Ages for PHC group,cirrhosis group,liver benign lesion group and healthy control group are 62.24±13.27(30-88)ages,62.23±14.58(35-82)ages,60.93±13.99(36-81)ages,59.97±16.32(33-82)ages,their age differences were not statistically significant(P>0.05).Levels for AFP,GPC-3 and GP73 of PHC group were 551.50±132.04ug/ml,18.55±14.67ug/l,198.20±99.45ug/ml respectively;Levels for AFP,GPC-3 and GP73 of cirrhosis group were 169.06±194.93ug/ml,9.02±5.28ug/l,113.60±52.38ug/ml respectively;Levels for AFP?GPC-3 and GP73 of liver benign lesion group were 16.92±38.03ug/ml?6.00±2.10ug/l?68.05±47.85 ug/ml respectively;Levels for AFP?GPC-3 and GP73 of healthy control group were 3.43±2.06ug/ml?4.09±2.21ug/l?66.32±45.11ug/ml respectively;Levels for AFP?GPC-3?GP73 of serum in PHC group was higher that that of cirrhosis group,liver benign lesion group and healthy control group,their differences were statistically significant(P<0.05),levels for AFP?GPC-3?GP73 of serum in cirrhosis group was higher that that of liver benign lesion group and healthy control group,their differences were statistically significant(P<0.05),differences between liver benign lesion group and healthy control group were not statistically significant(P>0.05).Sensitivity of serum AFP?GPC-3?GP73 diagnosed separately was 52.00%?58.00%?78.00% respectively,specificity was 92.22%?87.78%?91.11% respectively;sensitivity diagnosed for three was 96.00%,specificity was 75.56%,sensitivity of PHC diagnosed for three was better than that diagnosed separately,their differences were statistically significant(P<0.05).Differences in specificity were not statistically significant(P>0.05).In examination for patients with AFP negative PHC primary liver cancer,sensitivity of GP73 was 75.00%,GPC-3 was 58.33%.Conclusions(1)The levels of AFP?GPC-3 and GP73 in the PHC group were significantly higher than those in the cirrhosis group?benign liver disease group and the healthy control group,that is,the level of the three indicators in serum was increased with the appearance of primary liver cancer and the development of the disease.(2)In a separate test,GP73 is superior to AFP and GPC-3 in diagnostic sensitivity of PHC;(3)Relevance ratio of PHC for combining AFP,GPC-3 and GP73 was higher than that examined separately,therefore,combined detection of three indicators can improve diagnosis of PHC in clinical.(4)In patients with AFP negative hepatocellular carcinoma,GPC-3 and GP73 have good diagnostic sensitivity,which can make up for the lack of AFP diagnosis.
