Research background:Gunshot wound which harm to human beings has never terminated, and high speed bullet injuries on the body are also more complex. On the one hand the primary ballistic damages the body, including bleeding, burns, pain and systemic stress reaction. On the other hand cavity pressure around the gunshot wound in the trajectory formation of transient,can triggere disturbance waves in blood vessels and damage distant organ injury. Although distant organ injury mechanism induced by the gunshot wound is not entirely explained, the distant organ injury and the following two issues are included. First the organization can absorb the energy of injury. previous study proved that the value of more than 367.6 joules of energy can occur, and the energy aggravated distant organ injury. Second distant tissues wound is related to gas-water content, viscosity and density. The lung is one of the loosest body tissue structure. And the cardiovascular system in an enclosed pipe accord with the original trauma road, instantaneous cavity pressure on the structural conditions of transmission. Thus, respiratory and circulatory system are most likely to be involved when gunshot wounds occured. Bad results might be followed,because the two systems are very important physiological organ.In recent years, the application of 64-slice CT enabled the examination of non-invasive heart and lung morphology and function and entered a new period. The paper provided the reliable theory basis for gunshot wounds of the mechanism with the examination of stress response related hormone and blood vessel active material in blood plasma and comprehensive analysis of porcine pulmonary circulation in 64-slice CT and biochemical changes in the characteristics of plasma before and after hindlimb gunshot wound.Objective:Evaluation on the lung perfusion by the gunshot penetrating wound in pig's hindlimb with 64-slice VCT gunshot pig's hindlimb penetrating wound at different time points before and after the bronchus and the pulmonary vascular changes, combined with hormones of stress and vasoactive substances at each time point detection.Methods:12 female swines (3-6m,30-50kg) were provided by the animal century of the Fourth Military Medical University. The animal bullet wound model were made on hind limb by ilitary-style rifle 81 (wood holder)and bullet 7.62mm.64-slice VCT was applied on conventional sweep and contrast enhancement on chest at before shotting and after wound made 3min,10 min,25min,40min.Then the bronchus, lung blood caliber value and the vessel volume were analysed by VCT AW4.2 at different time.Meanwhile, COR,ET, T-AOC,MAD and SOD in plasma were detected,which taken from ear vein.Then analyse the relation between biochemical indicator and the change of different classes of the bronchus, lung blood caliber value and the vessel volume.Results:Through the LSD test,1.there was no significant difference in 1-5 level of lung blood vessel and bronchus (P>0.05).2.After gunshot 3 minute,vessel volumes of lateral basal segment of two lower lobes were significantly higher than those of before gunshot(P<0.05). After gunshot 10 minute, volumes were reduced obviously(P<0.05). After gunshot 25 minute,vessel volumes of lateral basal segment of two lower lobes were significantly higher than those of gunshot 10 minute(P<0.05). There was no significant difference in the shooting of blood biochemical values at gunshot 40 minute and 25 minute (P>0.05).At gunshot wound 25 minute and 40minute, vessel volumes of lateral basal segment of two lower lobes were restored level of gunshot wound (P>0.05).3.There were no significant difference in total antioxidant capacity (T-AOC), Superoxidedismutase(SOD),Malonyldialdehyde(MAD)and Endothelin (ET) (P>0.05).4.At the gunshot time of before and after, the level of ET and COR in blood plasma were significant difference among the different time (P<0.05),but it achieved the highest level at gunshot wound 10minute.Conclusions:1,Enhanced 64-slice CT scan and reconstruction technique can display and analysis of pulmonary vascular diameter and pulmonary vascular volum before and after injury.2,1-5 grade of blood vessels in the wound diameter before and after trauma 3min, 10min,25min and 40min each time point didn't changed obviously with the measurement and analysis of 64-slice CT image.But at 3min after shotting, pulmonary vascular volume increased significantly.Then declined slowly and at 25min,which recovered to the normal level.3,The changes of the pulmonary vascular volume and plasma ET, COR showed the opposite trend before and after injury, and the plasma T-AOC, SOD, MDA had no specific changes.
