Purpose: It is more difficult to diagnose small breast cancer with diameter 2cm with 2-Dimentinal sonography examination . In this study ,small breast cancer is researched in Color Doppler Flow Imaging(CDFI) and Color Doppler Energy(CDE),to explor blood flow feature and improve the diagnose target and offer more accurate diagnose target in sonography examination for diagnose small breast cancer earlier.Methods: One handred and twenty small breast masses were studied. Small breast cancers and benign breast lesions each held 60 cases. A instrument furnished with computed color-coded doppler sonography system and a 10 MHZ linear-array transducer was used in this study. All subjects lay on their back and were observed with direct contact. The breast lesion was first scanned in 2-Dimentional sonography examination, including its shap,edge,size,echo and lateral shadow. Then, the color flow which internal and peripheral of the mass was observed with Color Doppler Flow Imaging(CDFI) and then with Color Doppler Energy(CDE). The number of internal and peripheral vessels were calculated after the scan was found had the most abundant blood flow signals and the signals were classified. At last ,nature of flow signal was detected by spectral Doppler Ultrasonography ,and the peak systolic velocity (PSV) and the resistance index (RI) were measured . The mesures were performed for three times and the average parameters were recorded . Some patients were also examined with mammagraphy. They were diagnosed by ((The standard of diagnosis and treatment of breast cancer}} .Results: The blood flow signals were rich in small breast cancer. Among 60 small breast cancers , the type III-IV flow signal wasIVdemonstrated 51; occupying 85% of cancers . Among 60 patients with benign breast masses , type III only in 8 , occupying 13.3%. The signals were significant difference between the malignancy and the benignancy. The characters of blood pattern of small breast cancer include : 1) penetrating blood flow . 2 ) folding and transfering blood flow. 3 ) furcate blood flow. Although the PS V of breast cancer were difference with that of the benignancy , there was no significant diffierence. Only use RI^ 0.67 as a criterion to diagnose small breast cancer , the sensitivity was 92% and the specificity was 86% and the accuracy was 85% . But when we use above type III flow signal and RI^0.67 as a criterion to diagnose small breast cancer , the sensitivity was 94% and the specificity was 89% and the accuracy was 91.7% . There were 43 of 60 small breast cancers and 32 of 60 small benign breast masses also were examined with mammagraphy, the sensitivity was 86% and the specificity was 87.5% and the accuracy was 86.7%. The positive diagnostic rate of 43 small breast cancer was 92.8%, whereas the positive diagnostic rate of mammagraphy was 86%. The positive diagnostic rate of mammasonography combined with mammagraphy was increased to 97.7%.Conclusion: The manifestations of small breast cancer in 2-Dimentinal sonogyaphy were not most typical, so we ought to diagnosing with Color Doppler Ultrasonography . We can more accuratly diagnose small breast cancer with character blood flow in CDE and Pulsed wave Doppler . It is more accuratly diagnose small breast cancer when use ultrasonography and mammagraphy ,and serving for clinical treatent.
