Primary bronchial lung cancer is one of the most common malignant tumors. Among which,80%are non-small-cell lung cancer(NSCLC).80%of patients are in the advanced stage(stageⅢB or stageⅣ).Its 5-year survival rate is less than 10%. With the progress of the times,especially in recent decades,the participation of traditional Chinese medicine(TCM) in the treatment of advanced NSCLC manner, methods and results achieved are ongoing change and improvement.This study attempts to recall advanced NSCLC patient medical records participated in the treatment of TCM in our department of Oncology in the last decade(1998.1.1 -2007.12.31).This study wants to analysis medical records in the basic situation of case treatment program,short-term and long-term effect,changes in economic indicators,between in the past five years(2003-2007) and the first five years (1998-2002),between TCM treatment and integrated TCM and western medicine (WM) treatment.This study also attempts to reveal the trends of TCM treatment of advanced NSCLC,as well as to provid the useful information on standardization of clinical treatment and improvement the curative effect.Information about this study come from Guang'an Men Hospital of China Academy of TCM.The author collected 262 cases of advanced NSCLC cases. Among which,117 belong to the first five years(1998-2002) group and 145 belong to the past five years(2003-2007) group,150 belong to integrated TCM and WM group and 112 belong to the TCM group.Results show:(1) In general,the 262 cases of advanced NSCLC cases in our hospital in the last decade coincidenced epidemiological situation,but it shows the downward trend in age and the increased trends in proportion of adenocarcinoma.qi xu tan shi disease syndrome is the most common type;The rate of TCM treatment is 100%. The most common approach were TCM decoction,oral chinese patent medicine and TCM injections.There were an average of 2.8 kinds of approaches.The new chemotherapy programs accounted for 3/4,and both the efficacy and economy; Due to many non-initial treatment,treatment efficiency is lower than reported in the literature.Also the clinical benefit rate got 76.7%,and the treatment of patients take a better stabilizing role on quality of life.The median TTP reached 5.21mon,and the one-year survival rate reached 50.8%.(2) The factors influencing prognosis of these patients with advanced NSCLC were integrated TCM and WM treatment,anemia,treatment of the past five years (2003-2007) group by the Cox regression analysis.Integrated TCM and WM treatment and treatment of the past five years(2003-2007) group is the protective factors.Anemia is the risk factors.(3) Although the baseline was better than in the past five years(2003-2007) group to in the first five years(1998-2002) group about the male percentage and the visceral metastasis percentage(P<0.05),it achieved a better Clinical benefit (P<0.05),the higher rates of a stability quality of life(P<0.05),longer TTP (P<0.05) in the past five years(2003-2007) group by more TCM treatment and updated chemotherapy treatment(P<0.05).At the same time it was with less hospitalization time,drug costs,total hospital costs and better cost-effectiveness ratio.(4) According to the general treatment groups,the baseline in TCM and WM group was better than in the TCM treatment group about KPS score slightly (P<0.05).After treatment,it got a significant increase in efficiency and clinical benefit rate(P<0.05),a significant improvement of TTP(P<0.05) and better cost-effectiveness ratio.At the same time,it got richer manner of TCM treatment (P<0.05),and better stability rate in the disease(P<0.05),trends of higher rates of a stability quality of life,less hospitalization time,drug costs and total hospital costs.
