Nowadays, the mental health service research on disabled students in schools is mainly done from the perspectives of psychology and pedagogy, The research on the disabled students'mental health service from the perspective of social work is still seldom done. Taking the Yunnan H secondary specialized school as the research subject, and based on the investigation of the main psychological problems, the understanding and the need of mental health services of these disabled students and the mental health services of H secondary school, combine the related theory of mental health, and then construct the effective mental health service model for disabled students from the psychological demand of the disabled students.This paper is a research of disabled students'mental health service practice based on action research, which aims to improve the psychologically health of students with disabilities,enable these students to better integrate into society and live a healthy, meaningful and self-fulfillment life, according to solve the concrete psychological problems of theirs, Through the mental health service carried out by raising the abilities to adapt of the disabled students, establishing and expanding the social support network and capacity building. Finally, this paper reflects the disabled students'mental health service model and also the aspects to be improved in the future.
