Acupuncture-type High-frequency Hearing Loss Clinical Observation Of Sudden Deafness

Objective:By analyzing the hearing loss degree of the patients with sudden deafness and its relationship with TCM syndrome differentiation, and comparing the changes before and after the two respective treatments of patients suffered sudden deafness caused by wind-heat invasion, mainly the ones with high-frequency hearing loss, namely acupuncture combined with medicine, and pure west medicine, it has been found out that the treatment-acupuncture combined with medicine on the sudden deafness triggered by wind-invasion, particularly high-frequency hearing loss has the remarkable curative effects, thus providing the solid empirical basis for diversified treatment methods and therapy evaluation.Method:1 Select the patients with sudden deafness who had been diagnosed and hospitalized in Acupuncture Department or ENT Department in Fujian Provincial Hospital from November 2009 to November 2010. Based on the diagnostic criteria, inclusion criteria, exclusion criteria and the evaluation standard, the information about 61 patients with typical symptoms concerning wind-heat invasion is collected, whereafter they are divided into two groups, with 30 patients constituting one group treated by acupuncture together with medicine, and the rest 31 treated purely by west medicine. Pure tone audiometry are carried out on the patients in two different groups so that the features regarding the results of audiology test are analyzed.2 Points chosen for the group treated with acupunctured and medicine:Ermen, Tinggong, Tinghui, Yifeng, Zhongzhu. Points to be injected:Ermen, Tinggong, Tinghui, and these three points will in turn be injected with methylcobalamin lml once for a day, with 14 times forming one course of treatment. The patients in other group are dosed with incretion (dexamethasone) and microcirculation drugs (Ginaton).The dosage of dexamethasone is l0mg/d for the first three days, being reduced to 5mg/d on fourth day and fifth day, still being lessen to 2.5mg/d on sixth day and seventh day, and zero dosage on the eighth day; and the dosage of Ginaton is 70 mg for consecutive 14 ten days. After one course of treatment, the pure tone audiometry is carried out to record the curative effects.3 This study has the following observations and assessments:①Chinese Symptom Assessment;②the assessment of threshold audiometry;③the clinical efficacy evaluation of the treatment;④To compile the respective statistics of two threshold audiometry distribution before and after the treatment.Results:130 cases are included in the first group treated both by acupuncture and medicine, (30 ears), including 20 patients with tinnitus, and 14 cases with vertigo; the pure-western-medicine-group is made up of 31 cases (31 ears), within which 24 patients are diagnosed with tinnitus and 16 patients with vertigo. Two average thresholds of threshold audiometry have remarkable differences, and the hearing loss of the patients in these two groups are almost the same, mainly at mild to moderate degree of hearing loss, and their hearing graphics are basically associated with high-frequency hearing loss.2 The clinical observation results of the two groups:after one course of treatment, the acupuncture-and-medicine group has the 36.67% of cure rate,20.00% of markedly effective rate, 30% of efficiency and 86.67% of the totally effective rate. The pure-west-medicine group has 19.35% of cure rate,16.13% of markedly effective rate,25.81% of efficiency, and 61.29% of total efficiency. The curative differences of these two groups have great significance after being tested by statistics (P<0.05) and it is obvious that the treatment methods of the acupuncture-and-medicine group excels the methods adopted by the Pure-west-medicine group.3 After the treatments, the simultaneous phenomenon associated with vertigo has been lessened notably compared with it before the treatment while symptoms caused by tinnitus remains almost the same. The threshold audiometry of the two groups have great differences before and after the treatments. After the treatment, the electric audiometry of the acupuncture-and-medicine group is reduced more remarkably than the Pure-west-medicine group.Conclusion:1 Preliminary view is that wind-heat invasion-type hearing loss in patients with sudden deafness and its graphics are mainly about high-frequency hearing loss, which provides the clue that there exists a certain relationship between different frequency hearing losses examined by audiometry and TCM syndrome differentiation.2 The acupuncture combined with medicine and the pure west medicine, these two therapies both can ameliorate the syndromes of patients with sudden deafness. However, acupuncture combined with medicine can elevate the indicators mentioned above faster.
