Objectives:With high incidence, low cure rate, and high recurrence rate, chronic osteomyelitis is a common form of clinical disease. Salvia miltiorrhiza Bunge. f. alba. (SMB) is a labiates perennial herb with pharmacological effects of promoting blood circulation, detumescence and analgesic, nourishing blood and tranquilizing mind, anti-tumor, anti-oxidation. The aim of the experiment is to establish chronic osteom-yelitis model, observe the therapeutic effect of the aqueous extract of SMB decoction on lavaging chronic osteomyelitis and explore its mechanism in rat model, and lay a theoretical and experimental basis for the application and further development of SMB.Methods:Solution of staphylococcus aureus with restored toxicity and 5% Sodium Morrhuate (SM) were injected into rats'tibias after 38 SD rats were anaesthetized by injection ketamini hydrochloride. Modeled SD rats confirmed by X rays after 4 weeks were divided randomly into 3 groups:the aqueous extract of SMB group, antiseptic solution group (AS group) and normal saline group(NS group). Every rat was lavaged by the aqueous extract of SMB, AS or NS according to different groups. All were treated for 6 weeks. The following indexes were observed:serum lysozyme activity, WBC accounting, erythrocyte c3b receptor ratio (E-C3bRR), and erythrocyte immune compound ratio (E-ICR), and bone X-rays and histopathologic examination.Results:1. The results indicated that the blood leukocyte count of rat serum descended gradually in NS group while increased in SMB group. Great significant contrasts were found between the other groups and the NS group on the 28th day, (P<0.01).2.4th weeks after modeling red blood cell C3b receptor rosette rate and immune complex rosette rate of red blood cell and serum lysozyme content and blank control group were significantly different. After modeling, red blood cell C3b receptor rosette rate and immune complex rosette rate of red blood cell and serum lysozyme levels were higher than blank control group, suggesting that Chinese medicine can enhance immune function in rats.3. SMB group rats were in slight symptoms, AS group rats in moderate symptoms, while the control group rats in severe symptoms. SME group and AS group were better than the control group. SMB group has no significant difference with SMB group in contrast. Light microscopy found that:pathological changes of SMB group rats mainly were fibrous hyperplasia. Focal necrosis, significant proliferation in fibrous tissue, capillary surrounding trabecular bone, and inflammatory cells attaching to walls or swiming out around the blood vessel wall, new bone tissue and osteoclasts inside the medullary cavity could be seen under optical microscope; Pathological changes of AS group rats were mainly granulation tissue. Under optical microscope, we could see only a small amount of blood vessels in granulation tissue, a small amount of sequestrum and trabecular bone structure, inflammatory cells, new bone tissue, osteoclasts within the medullary cavity, but no obvious dead space; Pathological changes of the control group rats showed the coexistence of hyperplasia and destruction. Dead cavity and sinus appeared partially, arrangement of trabecular bone structure disordered or disappeared, necrosis of bone formed, mother cells, bone cells and broken Giant cell reduced in the number, and it was substituted by a large number of lymphocytes, plasma cells, small blood vessels with large areas of congestion, and inflammatory necrosis.4. Superior to AS group, SMB group had significant contrasts with blank control group. At the same time, AS group was better than the blank control group. Comparing X-ray results, we could see that the SMB group mainly was in slight symptom. X ray found high-density lesion area, marrow cavity in recanalization, neat bone configuration and trabecular bone structure. AS group was mainly in moderate symptom, and X ray found hyperplasia bone, narrow or disappearing marrow with thicker and irregular appearance, no sequestrum or sequestrum of bone surrounded by cladding. Control group was mainly in severe symptom. X ray found significant bone destruction, marginal no sclerosis, obvious periosteal reaction, high density of necrotic bone imaging.Conclusion:The therapeutic effect of aqueous extract of SMB decoction on lavaging chronic osteomyelitis in rats is more effective than the control group. Its possible mechanism are:1. Anti-drug and detoxification, protecting the body's defense function, inhibiting the development of inflammation; 2. To improve the blood circulation and enhance the body's repair functions; 3. To improve the body's immune function; 4. Repair function on bone and muscle injuries.
