Analysis On Diabetic Retinopathy Of Patients With Type2End-stage Diabetic Nephropathy

In the past studies, the relationship of diabetic retinopathy and diabeticnephropathy is confirmed. But there are also some patients with severeproliferative diabetic retinopathy exempt from diabetic nephropathy and somewith end-stage diabetic nephropathy lack in diabetic retinopathy. These specialpatients lead a new research direction for the association between the severityof diabetic nephropathy and diabetic retinopathy.Objective: To evaluate the progress of diabetic retinopathy of patientswith type2end-stage diabetic nephropathy.Methods:31patients treated with hemodialysis regularly for type2end-stage diabetic nephropathy were given a comprehensive dilated eyeexamination in the hemodialysis center of Second Hospital of Jilin University,whose clinical data were collected. According to the progression of retinopathy,patients were divided into normal group(NDR), nonproliferative diabeticnephropathy group(NPDR) and proliferative diabetic nephropathy (PDR). Theprogression of diabetic retinopathy and information including gender, age,blood pressure, treatments, lineal relative with diabetes, duration of diabetes,nephropathy and hemodialysis were analyzed by Chi-square test.Results: The mean age of31observers(male18, female13) was58.55and74.19%was between50-65. In60eyes(2eyes are traumatic atrophic) therewere9eyes without retinopathy,17eyes with I stage of retinopathy,10with Ⅱstage,6with Ⅲ stage,1with Ⅳ stage,11with Ⅴ stage,6with Ⅵ stage, and70.00%of which were not proliferative. There were5patients in NDR,15inNPDR and11in PDR with the rate of6.13%,48.39%and35.48%respectively.The longest duration of diabetes was30years and the shortest was3years. There were14patients with mean arterial pressure(MAP)≤105mmHg and17with MAP>105mmHg. The longest duration of nephropathy was13years andthe shortest was5months. The longest duration of hemodialysis was4.5yearsand the shortest was2months. There were17eyes dealed with treatments and13of which were given laser photocoagulation and3with vitrectomy. Thenumber of patients with at least one lineal relative was13. The gender, age,blood pressure, treatments, lineal relative with diabetes, duration of diabetes,nephropathy and hemodialysis in three groups did not show any difference.Conclusion: When it comes to the end-stage diabetic nephropathy, anyseverity of diabetic retinopathy is available, even absent in some cases. It issuggested that diabetic retinopathy fails to share a consistent progress with thetype2end-stage diabetic nephropathy. The gender, age, blood pressure,treatments, lineal relative with diabetes, duration of diabetes, nephropathy andhemodialysis are not associated with diabetic retinopathy.
