Research BackgroundBreast cancer is one of the most common malignant in women. At present, over one million women were diagnosed as the breast cancer in the global every year, which accounts for about25%of the women’s cancer, at the same time, the incidence of the breast cancer is of an upward trend year by year. In China, breast cancer accounted for7%~10%in various malignant tumor of the whole body. China as the most populous country of the world, the incidence of the breast cancer show clear ascendant trend too, and become one of the fastest growing country with the incidence of the breast cancer, some big cities have put the breast cancer as the first of women malignant tumor, and patients age tends to younger.Ki-67exist in cell nucleus is a nonhistones nucleoprotein, which gene location in chromosome10, that is related to cell proliferation, which can accurately reflect cell proliferation activity, and that was used to assess the tumor biological behaviour. Too much studies show that the expression of Ki-67can reflect proliferative rate of malignant tumor accurately, and related to the development, transfer, and prognosis of tumors. Because of Ki-67can be used as proliferating cell nuclear antigen(PCNA) of lots of tumor such as lung cancer, gastric cancer, nasopharyngeal cancer, rectal cancer,colon cancer, and breast cancer. Evaluation of Ki-67expression may have important values for study of biological behavious, judgement of harmfulness, guideline of treatment, and prediction of prognosis. Thus, Ki-67expression has been used as the clinical application of pathology index, in NCCN guide of revised edition of2011. At present, Ki-67is indispensible in clinical pathological diagnosis report of the breast cancer, definiting the correlations among Ki-67and histologic types, tumor size, lymph node metastasis and pathological immunohistochemical index (ER, PR, Her-2and P53) are very important of clinical value and guilding significance.ObjectiveThe aim of this study is to investigate the association between clinicopathologic parameters and Ki-67expression in primary breast infiltrating ductal carcinoma and assess its clinical significance.Methods123breast cancer samples were collected from Breast Surgical Department of Qilu Hospital of Shandong University from April2011to February2012. Among these samples, there were20tri-negative specimens. They were all did not accept chemotherapy, radition therapy, endocrine therapy and targeted therapy at preoperation. After tumor resection in surgery, expression of Ki-67and other clinicopathologic factors in123cases of breast cancer were assessed by immunohistochemical methods and their correlations with clinical and pathological factors were analyzed; at the same time, to independently investigate Ki-67expression in20cases of triple-negative breast cancer and their correlations with clinicopathologic parameters.Results1. The positive expression of Ki-67was92.68%in breast infiltrating ductal carcinoma tissue, Ki-67expression was not correlated with patient’s age, tumor size and lymph node metastasis (P>0.05).2. Expression of Ki-67was correlated strongly with histologic classification of tumor (P=0.004, P<0.01).3. Ki-67expression showed negative correlation with estrogen receptor(ER)(P=0.023, P<0.05) and progestin receptor (PR)(P=0.016,P<0.05); positive correlation with CerbB-2(P=0.016, P<0.05)and P53(P=0.036, P<0.05).4. Tri-negative breast cancer were found in20(16.26%) out of123patients. Expression of Ki-67was more commen in tri-negative breast cancer (16:20,80%) than in non-tri-negative breast cancer (53:103,51.45%; P=0.019, P<0.05).Conclusion1. Ki-67expression was higher in breast cancer, and its expression was more higher in tri-negative breast cancer than in non-tri-negative breast cancer.2. Expression of Ki-67was correlated significantly with the histologic classification in primary breast carcinoma, and there was no correlation with patient’s age, tumor diameter and lymph node metastasis status.3. Expression of Ki-67was correlated with ER, PR, Her-2and P53, and negative correlation with ER and PR; positive correlation with Her-2and P53.4. Ki-67expression can be used as a biological marker for estimating the development and progression of breast infiltrating ductal carcinoma, combined detection of Ki-67and other clinicopathologic factors of breast carcinoma may help clinicians to understand biological behaviour, guidance of breast cancer treatment, and evaluation of prognosis.
