Objects:To study the expression of CK19mRNA and Survivin mRNA in peripheral blood of colorectal cancer(CRC)and to explore the significance of the combined detection in colorectal cancer surgery to judge hematogenous micrometastasis of tumor cells and the method of operation, then to study the influence of laparoscopic resection of colorectal cancer on micrometastasis of tumor cells and comparison with open surgery.Methods:CK19mRNA and Survivin mRNA were detected in peripheral blood of28patients with open radical resection of colorectal cancer and25colorectal cancer patients underwent laparoscopic radical resection of colorectal cancer by RT-PCR.Results:①The preoperative positive rate of CK19mRNA、Survivin mRNA were35.8%(19/53),37.7%(20/53)respectively, the positive rate of combined detection was66.0%(35/53),there were significant differences between the individual and combined detection (P<0.05).②Positive rate of CK19mRNA and Survivin mRNA in peripheral blood is relate with tumor stage and lymph node metastasis(P<0.05),but has no significant relationship with tumor differentiation and tumor location(P>0.05).③The expression of CK19mRNA and Survivin mRNA have no difference between preoperative and postoperative in peripheral blood in laparoscopic surgery group, the expression of CK19mRNA and Survivin mRNA also have no difference between preoperative and postoperative peripheral blood in open surgery group (P>0.05).Positive rate of CK19mRNA and Survivin mRNA have no difference between laparoscopic surgery and open surgery group in before surgery,after surgery and changes of preoperative and postoperative (P>0.05).Conclution:1. Detection of combined CK19mRNA and Survivin mRNA improves the positive detection rate of colorectal cancer cell micromeatastasis. 2. The results of the expression of CK19mRNA and Survivin mRNA show that the positive rate of free cancer cells in preoperative peripheral blood is relevant to tumor stage and lymph node metastasis, suggests that cancer cells may be one of the prognosis factors of the colorectal cancer patients.3. Compared with open surgery, laparoscopic resection of colorectal cancer may not increase the risk of hematogenous micrometastasis of tumor cell. However the reliable conclusion require further study.
