Clinical Study On The Effect Of Polycystic Ovary Syndrome With Insulin Resistance By The Method Of Invigorating The Kidney And Regulating Menstruation Cycles

Objective:This study used the method of tinvigorating the kidney and regulating menstruation cycles to treat polycystic ovary syndrome with insulin resistance(IR-PCOS). Before and after treatment, it mainly observed the changs of some related indexes, such as, patients’ menstrual cycle, BBT, TCM syndrome marks, hairy and acne score, B-ultrasound examination of ovarian follicles, the levels in the serum of T, SHBG, E2, LH, FSH, PRL, FBG, FINS, the BMI and Homa-IR indexs.According to the result, we estimated the curative effect of improving patients’ clinical symptoms and menstruating after ovulation by using the method of invigorating the kidney and regulating menstruation cycles. In contrast with the therapeutic schedule of only metformin or adding it, we observed the treatment effect about the method of invigorating the kidney and regulating menstruation cycles used for IR, and how effective the traditional Chinese medicine combined with western medicine would be.On this basis, doctors will have a well-founded choice of Tcm therapy, western medical therapy, or the united one to cure IR-PCOS.Method:In this study, in the gynecological clinics at Jiangsu Province Chinese medicine hospital and in the department of Endocrinology about PCOS at Gulou Hospital, we Collected60patients who met inclusion criteria from March2011to March2012.All of them were Randomized into3groups:TCM group(Group A),Western medicine group(Group B), TCM Combined with traditional group(Group C).Patients in Group A were treated with the method of invigorating the kidney and regulating menstruation cycles.and given syndrome differentiation and treatment with traditional Chinese medicine in different menstrual period.Ones in Group B were treated with Oral metformin.Ones in Group C were treated with both of the therapies. A period of treatment is3months.Before and after being treated for6months,patients should be draw peripheral vein blood for test of T、SHBG、E2、LH、FSH、PRL、FBG、FINS on an empty stomach on the second or third day of their menstrual cycle, check Gynecological B-ultrasound examination and Weigh the latest weight.During this transfer,we helped them calculate the BMI and Homa-IR indexs, observe the changes of their BBT and the improvement of symptoms about menstrual cycle,acne, hairiness, and so on.Results:the total efficiency (95%)of TCM Combined with traditional group was significantly higher than TCM group (85%) and Western medicine group(85%).TCM Combined with traditional group was significantly different from Western medicine group in the curative effect(P <0.05).Both of TCM Combined with traditional group and TCM group were all effective to improve the patients’ TCM syndromes, and superior to western medicine group (P<0.05).All the patients in these three groups had a shorter menstrual cycle and acne improvesments,while there were no difference in three groups(P>0.05).At the same time,three groups had no obvious Improvement of the BBT and hairy symptoms(P>0.05).Aftertreatment,serum T、FINS levels and Homa-IR indexs declined in three groups.All of them had significantly statistical differences (P<0.05).The above indexes also had statistical differences Among groups(P<0.05).TCM Combined with traditional group and Western medicine group took more remarkable effect on the Indexes of serum T、FINS,and LH/FSH、Homa-IR indexs.Making a general survey of the materials before and after treatment between three groups,we made a correlation analysis.The result forecasted that the level of Homa-IR was positively correlated with patients’ weight,BMI，FBG,T,E2level was positively.and highly yet positively related with FINS level.On the other Side,the level of Homa-IR was inversely related to the level of serum SHBG,pretherapy and post-treatment.Conclusion:The method of invigorating the kidney and regulating menstruation cycles combined metformin had a definite effect on patients of IR-PCOS,and significantly reduced the insulin resistance, balanced the Sex hormonesenvironment and endocrine environment in petients’ body.The associative therapy also improved patients’ menstruating after ovulation and clinical symptoms about amenorrhea and acne,which caused by hvperandrogenism.This study showd that the method of invigorating the kidney and regulating menstruation cycles or Oral metformin,each of them contributed to shorten the menstrual cycle,improve acne symptoms.The better curative effect of TCM group was the improvement of TCM syndromes.The better one of Western medicine group was the improvement of insulin resistance.TCM Combined with traditional group had the the best curative effect.
