Objective:To observe the clinical efficacy of treating chronic renal failure (decompensated) with syndrome differentiation and treatment of Chinese medical. To further understand the occurrence and development of chronic renal failure at the medium-term, as well as the effectiveness of the intervention of the Chinese medicine, thus, to overall grasp the disease, and to guide clinical treatment better.Method:With prospective clinical research method used, the outpatient medical records of39patients from October2011to January2013with chronic renal failure at decompensated period were collected. The patients were divided into different groups according to the syndromes, and given the corresponding decoction based on the conventional Western medicine treatment. A month as one course of treatment, the result evaluation was done after two courses of treatment. The related laboratory parameters of patients were detected in each course of treatment, including serum creatinine (Scr), blood urea nitrogen (BUN), glomerular filtration rate (GFR), and hemoglobin (HGB). TCM symptom changes of the patients were observed before and after treatment. The related laboratory parameters and changes of TCM symptom scores of the patients were observed before and after the treatment, and the clinical efficacy was statistically analyzed. And the patients treated for6months were analyzed during the treatment with the efficacy-the linear regression of Scr counting backward technique, so that the changes of the kidney function could be evaluated.Results:(1) The total efficacy to the disease:The total effective rate was71.79%; the total efficacy to patients with different CKD periods as follows:CKD3period with a total effective rate of83.33%; CKD4period with a total effective rate of81.25%; CKD5period with a total effective rate of45.45%. The above data show that the earlier period of the disease, the more sensitive to the Chinese medicine treatment, and the better efficacy. Therefore, seizing the early period of the disease to do intervention treatment for patients with CKD is the key to slow down the progression of renal function development.(2) The efficacy of TCM syndromes:Total stabilizing effective rate is82.05%. The TCM clinical symptom scores after treatment are significantly reduced than that before treatment (P<0.01). Therefore, it shows that syndrome differentiation and treatment of Chinese medicine has a significant efficacy to the patients with decompensated chronic renal failure.(3) Related laboratory indicators:Compared with pretreatment, Scr, BUN, GFR has been improved after treatment. After treating for4and8weeks, both Scr and GFR became better than pretreatment. with Scr decreased. GFR increased, and there was a significant difference (P<0.01). After treating for4weeks, BUN was decreased, and with a significant difference(P <0.05), while after8weeks, with a more significant difference (P<0.01). For HGB, there is no significant difference before and after treatment (P>0.05). It showed that the anemia of the patient was not obviously improved during the treatment.(4) The efficacy-the linear regression of Scr counting backward technique was analyzed during the treatment for the18patients with6month treatment and4times Scr detection. The results showed that:11patients were with a positive b-value and7cases with a negative b-value, and the overall b value of regression slope is positive, which showed that the general kidney function were tending to be better, and the syndrome differentiation and treatment of Chinese medicine can be used to improve the kidney function of the CRF patients, and effectively delay the CRF natural deteriorating tendency.Conclusion:In this research, syndrome differentiation and treatment of Traditional Chinese Medicine has an obvious efficacy to patients, and can be used to decrease the level of creatinine and urea nitrogen and raise glomerular filtration rate. thus, to effectively delay the progress of renal function. CRF is a chronic progressive disease, and the key of efficacy is perseverance.
