Objective: Explore the relationship of diabetic retinopathy and disorders ofglucose metabolism.Methods: To select community residents which attended the community generalinvestigation of diabetes mellitus in Dalian Central hospital among40years or olderfrom August162011to November102011. Totally10144complexing conditionpeople had been observed with clinical test data of non-mydriatic fundus photography.fasting blood-glucose. OGTT(oral glucose tolerance test) and glyhemcosylatedhemoglobin(HbA1c). Get rid of7788non-diabetics in the10144cases. According towhether they had diabetic retinopathy, the patients with2356diabetes mellitus caseswere divided into two groups:A and B group.A was the non-diabetic retinopathy(NDR) group, B was the diabetic retinopathy(DR)group. According to the stage ofdiabetic retinopathy, the non-diabetic retinopathy(NDR) cases were divided into B1group and B2group. B1group was proliferative diabetic retinopathy (PDR)groupand B2group was non-proliferative diabetic retinopathy (NPDR) group.Analyzingbetween the fasting plasma glucose (FBG),2h plasma glucose(2hPG).hemorrheology(HbA1c) and duration indicators in A and B group by t text,Logistic regression methods was used to analyze the correlation between the incidenceof different FBG and2hPBG in DR.through the results to evaluate the correlation ofdiabetic metabolic disorder with Diabetic Retinopathy. Segmentating the value of FBG,2hPBG andHbA1c and analyzing,than observing the correlation between that andincidence of DR.T text was used to analyze the correlation between the stage of DR andthe level of HbA1c in B1、B2groups. Multivariate logistic regression analysis wasused to analyze the correlation between FPG,2hPG, HbA1c, duration of diabetes,pressure, lipid and DR. Results:1. The fasting blood glucose value was8.42±2.66mmol/L in A groupwhile the value was8.89±3.49mmol/L in B group, the difference of the two groupswere not statistic significance (t=1.316,P>0.05). The fasting postprandial2h bloodglucose value was14.67±5.22mmol/L in A group while the value was17.75±5.69mmol/L in B group, the postprandial2h blood glucose value in the B was higherthan that of the A group (t=8.372,P<0.05). The HbA1c value in the B group(8.32±1.93%) was also significantly higher than that in the A group (7.17±1.89%)(t=8.664,P<0.05).The difference of duration of diabetes between A group (7.21±6.19)and Bgroup (8.69±6.61) had statistical significance(t=2.959, P<0.05).2. To analyze HbAlc value segmentation. The incidence of retinopathy indiabetics whose HbA1c>7%was higher than whose HbAlc≤7%by χ2test (χ2=43.77,P<0.05). Through analyzing level of HbAlc from5%to7%, the incidence ofretinopathy significantly increased when HbA1c>6.2%3. Compared the level of HbAlc in B1with B2group by t text of two independentsamples t=13.14, P<0.05, the difference of the two groups were not statistic significance.It illustrated the level of HbAlc was associated with the development of DR in diabetics.4. Logistic regression methods was used to analyze the correlation between eachrelative biochemical indicators, in which HbAlc (β=1.281, OR=2.756,95%CI2.308-5.586, P<0.05)、 duration(β=1.765,OR=5.111,95%CI4.373-7.742,P<0.05) and systolic pressure(β=0.487,OR=1.268,95%CI1.175-2.207, P<0.05)were significantly correlated to the incidence of retinopathy. It explained that HbA1c.Duration of diabetes and systolic pressure level are risk factors for the development ofDR in diabetics.Conclusion:1. As the level of HbAlc was higher, the prevalence date of DR increased. WhenHbAlc>6.2%,it increased significantly.2. These data suggest that HbA1c. Duration of diabetes and systolic pressure levelare risk factors for the development of DR in diabetics.
