Objective To observe the sex-specific relationship between cigarette smoking and theincidence of stroke in the Chinese adult population in Anqing and surrounding areas byexamining the interactions between smoking and the conventional risk factors. To studythe effect of cigarette smoking among spouses on the risk of stroke incidence amongChinese adult of Anqing.Methods The prospective study consisted of58,530Chinese who were enrolled from1995to2005at baseline survey. In2010and2011, all objects of baseline screeningwere invited to participate in a follow-up study. Stroke cases were confirmed by medicalrecord reviews. Cox proportional hazards model was used to evaluate the association ofsmoking with the risk of stroke and between spouses smoking with the risk of stroke.Results A total of26404patients were selected and completed the study the associationof smoking with the risk of stroke.During an average of9.5years follow-up, a total of509strokes were documented. The multivariate-adjusted relative risks (95%confidenceinterval) for current smokers compared with nonsmokers were1.46(1.18to1.82) fortotal strokes,1.35(0.89to2.06) for Cerebral infarction, and1.34(0.90to2.01) forintracerebral hemorrhage. The multivariate-adjusted relative risks (95%CI) of strokeincidence associated with present cigarette smoking were1.37(1.06to1.77) in men and1.88(1.28to2.77) in women, respectively. The risk of stroke associated with cigarettesmoking is higher in women. The corresponding population’s attributable risks were20.7%in men and7.3%in women. Stratified analysis results show that the risk of stroke associated with smoking are apparently far greater in older people, particularly age≥50years. A total of16706patients were selected and completed the study the associationbetween spouses smoking with the risk of stroke. During an average of7.0yearsfollow-up, a total of231stroke events were documented. Compared to people that bothhusband and wife are nonsmoking, the multivariate-adjusted relative risks (95%confidence interval) for interaction of husband and wife smoking were1.89(1.05to3.39)for stroke. The interaction between smoking and spouse smoking was statisticallysignificant (P=0.033). The risk was increased for strokes (RR,1.33;95%CI,0.93to1.90) among smoking people with a nonsmoking spouse, but the result was notstatistically significant. No significant correlation was observed between nonsmokingpeople with spouse smoking and stroke (P>0.05). The risks (95%CI) of strokeincidence associated with interaction of husband and wife smoking people were1.89(1.05to3.39) in men and1.04(0.30to3.64) in women.Conclusion The study identified a positive relationship between cigarette smoking andrisk of stroke. The risk of stroke associated with cigarette smoking is higher in women.This prospective study identified both husband and wife smoking increased the risk ofstroke incidence. Smoking prevention and cessation programs should be an importantstrategy for reducing the burden of stroke for Chinese adults in Anqing.
