Objective: The extracellular microenvironment is a dynamic entity,andit dictates the destiny of the cell by providing regulatory signals. Theregulatory signals can affect important cellular processes. Changes in thosefunctions may cause tumor. Matrix metalloproteinases are their majorregulators in the extracellu larmicroenvironment.The Matrix metalloprotein-ases are represent a zinc-dependent endopeptidase family with proteolyticalactivity on many components of the basement membrane and of theextracellular matrix. They participate in embryogenesis, angiogenesis,connective tissue reconstruction and immune and inflammatory processes.They also have a series of cell functions, and they are relevant to cancerbiology,such as cell growth and differentiation. The increasing level of MMPsexpression is a characteristics of malignant tumor, they are closely related withtumor invasion and metastasis.The matrix metalloproteinase (MMPs)including Mr72,000type IV collagenase (MMP-2, gelatinase A) and Mr92,000type IV collagenase (MMP-9, gelatinase B) play key roles in tissueremodeling and tumor invasion by digestion of extracellular matrix barriersand basement membranes. In acute leukemia (acute leukemia), matrixmetalloproteinase2and matrix metalloproteinase9are involved in somepathological processes, such as impaired bone marrow microenvironment,liver, lienal, lymph node enlargement, the central nervous systeminvolvement, and release a large number of immature cells into the bloodstreams. It causes serious prognosis. The aim of the article was to investigatethe gene of the two enzymes in ALL patients, deduce the relationship betweenMMP-2/MMP-9and the disease status of ALL, extramedullary infiltrates,peripheral blood or bone marrow blast counts or to the prognosis. We candeduce the matrix metalloproteinases may play a role of pathological effects and its mechanism in acute lymphocytic leukemia, and provides newtheoretical basis, provide new ideas and methods for its treatment.Methods: Real time quantitative PCR (QT-PCR) was applied to detectthe gene expression of MMP-2and MMP-9in marrow mononuclear cellsfrom healthy people and acute lymphoblastic leukemia (ALL) patients.Results:1The expression of MMP-2mRNA and MMP-9mRNA in24untreatedALL samples and5relapse samples had no significant difference, P>0.05.2The expression of MMP-2mRNA and MMP-9mRNA in24untreatedALL samples and16complete remission patients had significant difference,P<0.01.The expression of MMP-2mRNA and MMP-9mRNA in untreatedALL samples was higher than the remission patients.3The MMP-2and MMP-9mRNA in24untreated ALL patients washigher than that of the control group, the difference was significant, P<0.01.4The expression of MMP-2mRNA and MMP-9mRNA in5relapsesamples and16complete remission patients had significant difference,P<0.01.The expression of MMP-2mRNA and MMP-9mRNA in relapse sampleswas higher than the remission patients.5The MMP-2and MMP-9mRNA in5relapse samples was higher thanthat of the control group, the difference was significant, P<0.01.6The expression of MMP-2mRNA in16complete remission samplesand7healthy people had no significant difference,P>0.01.The MMP-9mRNAbetween the two groups had significant difference, P<0.01, TheMMP-9mRNA in16complete remission samples was higher than that of thecontrol group.7In order to confirm that the MMPs were one of the most importantreasons in Marrow infiltration, the patients were divided into two groups. theexpression ofMMP-2-positive cases and MMP-9-positive cases appearedextramedullary infiltrates in the patients of untreated and relapse patients washigher than, that of MMP-2-negative patients and MMP-9-positive cases, thedifference was significant, P<0.05 Conclusions:1MMP-2and MMP-9were exist in healthy people and the patients ofacute lymphoblastic leukemia, which is low in healthy people and highinuntreated and relapse patients. The expression The matrix metalloproteinases(MMPs) of the would be decreased after chemotherapy2A positive correlation could be found between the expression of MMPsand the extramedullaryinfiltrates. MMP-2and MMP-9could degradedextracellular matrix barriers and basement membranes and involved in theinvasion and the migratory of the leukemia cells and the destruction of thebone marrow microenvironment.
