Objective This topic by observing Tangshenkang particles of diabetic kidneydisease (DKD) Qiyinliangxu clinical efficacy in patients with blood stasis syndrome beforeand after treatment in peripheral blood mononuclear nuclear factor kB (NF-κB), nuclearfactor-κB p65subunit affinity peptide (NF-κBp65) and nuclear factor-κB inhibition ofcytoplasmic protein changes in α (IκBα), and to explore Tangshenkang particles of DKDpossible mechanism.Methods66cases in August2012-January2014in Anhui Province Hospital ofNephrology and DKD Qiyinliangxu blood stasis syndrome Endocrinology admitted wererandomly divided into treatment group and control group33cases, the treatment processlost three cases, two cases in which the treatment group1patients in the control group, andfinally the actual completion of63cases. And set the corresponding period in the normalgroup of20patients. Two groups of patients were given control of blood glucose, bloodpressure control combined therapy, diuretic; treatment group in the above treatment basedon the use Tangshenkang particles, each a bag,3times a day orally. The treatment lastedeight weeks. Were observed integral Qiyinliangxu stasis syndrome, urinary albuminexcretion rate and other indicators of change and adverse reactions; groups of patientsbefore and after treatment and to detect NF-κB, NF-κBp65, IκBα, detect only the outerperiphery of the normal group blood mononuclear cells NF-κB, NF-κBp65and cytoplasmicIκBα, determine the efficacy of the experimental results and statistical analysis.Results The clinical efficacy of the disease in patients treated with traditionalChinese medicine syndromes Yin Deficiency symptoms of blood stasis and major efficacywere significantly better than the control group, the difference was statistically significant(P <0.05or P <0.01). Two Qiyinliangxu patients with blood stasis syndromes points in thefour weeks before treatment was significantly lower than before treatment (P <0.05or P<0.01),8weeks of treatment compared with treatment also significantly reduced (P4weeks <0.01); and two earlier, the treatment group syndromes points declinesignificantly better than the control group (P <0.01). Two groups of patients treated for4weeks and8weeks proteinuria (UAER,24hUPr) before treatment were significantly improved (P <0.05or P <0.01); treatment for8weeks and4weeks of treatment comparisonalso showed significant differences (P <0.05or P <0.01); treatment group improved morethan the same period in the control group was significantly (P <0.01). After treatment, renalfunction (BUN, Scr, UA, eGFA) and glucose (FPG,2hPG, HbA1C) improvement trend,but not statistically significant (P>0.05). And the treatment group and the control group nosignificant difference (P>0.05). Two groups of patients before treatment NF-κB,NF-κBp65were significantly higher than the normal group (P <0.01), IκBα levels weresignificantly lower than the normal group (P <0.01), whereas no significant comparisonbetween the control group and the treatment group difference (P>0.05); after treatment,NF-κB, NF-κBp65levels before treatment were significantly decreased compared (P <0.05or P <0.01), compared to pre-treatment levels of IκBα were increased significantly (P<0.05or P <0.01), while the degree of improvement in the treatment group weresignificantly better than the control group (P <0.01). Pearson correlation coefficientanalysis showed: two groups of patients with urinary albumin excretion rate of NF-κB,NF-κBp65showed a significant positive correlation (r=0.631, P <0.01; r=0.592, P <0.01),and IκBα significant negative correlation (r=-0.624, P <0.01).Conclusions Tangshenkang particles can significantly improve symptoms inpatients with DKD Yin Deficiency of blood stasis, reduce proteinuria, with good clinicalefficacy. DKD patients with NF-κB, NF-κBp65, IκBα than normal healthy people havesignificant abnormalities, and with UAER was significantly correlated, suggesting thaturinary protein excretion in patients with DKD NF-κB protein levels associated anomaliesclose relationship. Tangshenkang particles can inhibit NF-κB, NF-κBp65, improve IκBαlevels, which may be one mechanism of clinical efficacy DKD Tangshenkang particletherapy.
