BackgroundVentricular premature beat is one of the most common arrhythmia. It’s incidence is high-er in normal people and various kinds of heart patients. When ventricular premature beat be-come seriously, it can cause hemodynamic disorder, and induce and aggravate cardiac insuffi-ciency, even death. Although significant progress been achieved in the anti-arrhythmic drugs and radiofrequency ablation on the treatment of ventricular arrhythmia and can part to control or cure, there are significant adverse reactions to these methods and still part of ventricular arrhythmia in clinical get few effects. In recent years, Chinese traditional medicine has a cer-tain advantage in the prevention and treatment of ventricular premature beat. After combining the theory of ancient and modern subject and on the basis of the experience himself, professor Wangxian combines traditional Chinese medicine with western medicine and explore the new ways to explaining the etiology and pathogenesis of premature. He found that the happen of ventricular premature beat is too frequent and changing in clinical, and it should be under-stood from the angle of "wind" which from the Chinese traditional medicine. So he put for-ward the theory of "Endogenous collateral wind" is an important pathogenesis of frequent ventricular premature beat. And the therapeutic principles of the theory is emphasized on the supplementing qi and nourishing yin, eliminating phlegm and tranquilizing, dispelling wind and removing obstruction in the meridians which are the theory of Chinese traditional medi-cine. And based the theory, professor Wang created Luofengning3, a new medicine to treating the frequent ventricular premature beat.ObjectiveThe aim of this study to evaluate the efficacy and safety of the Luofengning3for treat-ment of frequent ventricular premature beat, and to explore the influence on heart rate varia-bility in patients with frequent ventricular premature beat.Method61patients of frequent ventricular premature beat eligible for inclusion were chosen and randomly divided into treatment group and control group. The etiological and conventional treatment both given to all patients. And all patients stop using all kinds of anti-arrhythmic drugs (at least five T1/2S). Propafenone was given to30cases in control group under the dose of150mg,3times daily oral; Luofengning3was given to31patients in treatment group un-der the dose of1bag each time and2times daily oral. The course of treatment of both two groups is4weeks. Before and after the course of treatment, we observed clinical symptoms, TCM syndrome integral,24-hour dynamic electrocardiogram change, etc. of all patients. Ac-cording to different observation indicators and data, we were using SPSS17.0statistical soft-ware to statistical analysis, in which categorical data by chi-square test, measurement data by t test, rank grouping data using the corresponding statistical methods such as rank and inspec-tion process. Results1. Before the course of study, there was no statistical significance (P>0.05) on various in-dexes between two groups such as gender, age, pathogeny, Chinese medicine symptom inte-gral, heart rate variability, etc.2. Compared TCM syndrome improvement rate between the control group and treatment group after the treatment, the results show that the symptom score of TCM decrease obviously, and total effective rate of the treatment group and control group were90.3%and60.0%re-spectively, which shows the curative effect of treatment group is better than that of control group, with significant difference (P<0.05). About symptoms, the chest tightness, shortness of breath, fatigue, sudden onset, and recurrent symptoms have significant difference (P<0.05).3. After the course of treatment, the total number of ventricular premature beats was de-creased significantly at patients in both groups, compared with that before treatment (P<0.05), and no statistical significance between groups (P>0.05). What’more, the total effec-tive rate of two groups was77.5%and73.4%respectively.4. Compare of the HRV between treatment group and control group:(1) After treatment, the Standard Deviation of NN intervals (SDNN) both raised in two groups. By compare, SDNN in the treatment group is significantly higher than that of control group, with statistical signif-icance (P<0.05).(2)After treatment, the entire record of continuous5min sinus average Standard Deviation of the RR interphase (SDANN) both increased in both groups. By com-pare, there is no statistical significance between two groups (P>0.05).(3) After treatment, the whole period between adjacent sinus RR difference of Square Root of the mean squared differences (RMSSD) both increased in both groups. By compare, RMSSD in the treatment group was obviously higher than the control group, with statistical significance (P<0.05).(4) After treatment, the percentage of differences exceeding50ms between adjacent normal number ofintervals (PNN50) both raised in two groups. By compare, PNN50in the treatment group is significantly higher than that of control group, with statistical significance (P<0.05).5. In the course of study, there is no toxic and adverse reaction of Luofengning3was found.Conclusion1. Luo fan Ning No.3could significantly improve the clinical manifestations of endogenous collateral wind, indirectly confirms the endogenous collateral wind theory is one of the path-ogenesis of frequent ventricular extrasystole.2. The Luofengning3can reduce the total number of premature beat tested by24hours dy-namic electrocardiogram, and can relieve the objective indicator of patients.3. Luofengning3may inhibit sympathetic nervous activity of patients with frequent ventricu-lar premature beat. And it can improve the tension of vagus nerve relatively, thus improve the cardiac autonomic nerve function of patients with frequent ventricular premature beat.4. Luofengning3has no effect on lowest heart rate. And in the course of study, there is no toxic and adverse reaction of Luofengning3was found, which shows Luofengning3with good safety.
