Assessment On Indemnity Model Performance Of The New Rural Cooperative Medical System In Sangzhi County, Hunan

Objective:Research the problems in the process of running and the insufficiency and provide the basis and the suggestion for the further reform and improvement through the present condition analysis and comprehensive evaluation on the rural cooperation medical system in SangZhi county of Hunan.Methods:According to the research purpose, select the data from the rural cooperation medical system database in Sangzhi County of Hunan Province into Excel checked and analyzed the data. The satisfaction survey was adopted cluster random sampling method, divided the county30country in a total into3levels (good, average, poor)by the economic conditions, each level selected1randomly villages and towns as a research point and got three townships, each township randomly selected100patients, got a total of300people as objects finally. Analyzed the data with SPSS19.0and Excel3.1,and evaluated comprehensively the rural cooperation medical system from2010-2012by the TOPSIS and rank method of Li Yongqiu, Zhang Yingjie "compensation model comprehensive evaluation system of medical treatment of new-style country collaboration".Results:The Participation rate of the rural cooperation medical system had reached98%in SangZhi county of Hunan in2012, covered most of the rural population. The scale of the rural cooperation medical system fund-raising expands unceasingly and the declining savings rate was declining(at10.02%in2012)from2010to2012. The average hospitalization days increased to10.31days in2012; Outpatient and hospitalization expenses increased; Farmers who participated in benefited increased and the compensation rate also increased faster, outpatient compensation rate of58.17%and hospital compensation rate was61.86%in2012, accounting for farmers to pay and hospitalization expenses were decreased; The farmers generally chose township hospital hospitalization, accounted for about52%, followed by the county hospital, about35%, and in the city and the provincial hospital accounted for5%,6.5%.n SangZhi county.The rural cooperation medical system satisfaction survey results showed that, the patient’s satisfaction was good in total, selecting "very satisfied" and "satisfactory" accounted for78.45%."The doctor diagnosis and treatment level" and "hospital sanitation level" patients in the2answer satisfaction were low, at the rate of65.96%and63.56%respectively. Patients satisfaction to the hospital overall was good, but the doctor’s diagnosis and treatment level and health satisfaction were relatively low. In outpatient services fee "would you like to take the patient to pay the rest after a certain amount of expenses by new farming and fully reimbursed" answer, choose "yes" and "best" free of charge reached90.02%, the vast majority of farmers hoped that the clinic also enter in the rural cooperation medical systemThe effect of the rural cooperation medical system was the best in2012, followed by2011, and the effect of rural cooperation medical system was the worst in2010through the TOPSIS and rank method in SangZhi of Hunan.Conclusion:1.The reformed rural cooperation medical system mode improved the farmers hospital compensation proportion in the villages and towns, declined in both outpatient and hospitalization expenses in SangZhi Couty of Hunan Province.2. The reform of the compensation model was effective and successful overall according to the results of comprehensive evaluation through the TOPSIS and rank method.3. Farmers still at a high level costs in the county or above hospital hospital, the hospital medical treatment burden was still heavier.
