Medical insurance plays an important role in the social security system, it can protect people from suffering the loss of the right to health and reproduction capacity because of disease risks. Medical insurance can make the social more stable and can adjust income redistribution. Our country has established a universal system of basic medical insurance—Medical Insurance for Urban Workers, Medical Insurance for Urban Residents and New Cooperative Medical Scheme. The establishment of New Cooperative Medical Scheme plays an important role in reducing the burden on rural patients. However, the rural patients still face heavy burden of disease cost. So, Building a multi-level medical security system in rural areas has great practical significance to further ease the burden of large medical expenses. The paper analyzes the rationality and necessity of building the supplementary medical insurance for rural residents. Rationality:(1)The supplementary medical insurance comply with the insurance laws and can better reflect the risk-sharing principles;(2)It can protect the interests of the majority, it also be the welfare provided by country;(3)It also protect the human rights. Necessity:(1) The cost of catastrophic disease is so high and the disposable income of farmers is so low that lots of farmers can’t afford;(2)The high insurance deductible and low reimbursement rate is another reason.Through comparing three modes of the supplementary medical insurance in Taicang City, Chuxiong City and Yuxi City, the paper found some advantages, such as piecewise accumulative reimbursement ratio and no upper-limits in Taicang City, voluntary insurance principle in Chuxiong City, big financial support in Yuxi City and more. However, we also found some problem:the problem of fairness, finance issues, operation and management problem, and the clear relation with the basic medical insurance. So, we believe that the government should(1) establish the correct concept of development;(2)establish a stable long-term funding mechanism;(3)link the supplementary medical insurance with basic medical insurance more closely;(4) improve the relevant supporting measures. Finally, I believe that faced with complex situation, it is difficult to establish a unified model for government, so they need to establish a pilot mode according to local conditions. The establishment of catastrophic disease insurance for rural residents also should obey the principles of reforms—step by step.
