Objective: To discuss the operative procedures and clinical results of the posteriortibial plateau fractures using posterior approach of the knee.Methods:12cases with posterior tibial plateau fracture were treated with openreduction and internal fixation through posterior approach from October2007to April2014. There were8males and4females with the average age of47ranging from31to70years old. Of these12patients,8were left knees and4were right knees. All fractures werefresh and closed. There were4cases of posteromedial fractures,4cases of posterolateralfractures, and4cases of bilateral fractures. After operation, X-rays were taken in allpatients and complications were recorded. According to the results of the last follow-up,Rasmussen’s functional gradings were used to evaluate functional outcomes of knees.Results: All patients were followed up for12to77months with average of27.1months. All fractures got healed. According to Rasmussen’s functional grading, the meanscore of the knee joint was26.8(range,24to29).6were excellent,6were good.Excellent and good rate was100%. The range of motion of the knee joint ranged from0°to135°(average,119°). All the patients were satisfied with the clinical result. During theperiod of followed up, there was no loss of reduction; one case, who have been diagnosedwith injury of the common peroneal nerve preoperatively, presented with symptoms ofcommon peroneal nerve injury, such as local hypesthesia in distal lateral lower leg anddorsi pedis. No skin necrosis, infection, vascular injuries or screw loosening/breakage wasfound in the other cases.Conclusion: Posterior approach is an effective clinical method for treating posteriortibial plateau fracture, with the advantages of fully exposure, reduction under direct vision,firm fixation, minimal soft tissue dissection, and satisfactory postoperative results. However, surgeon need to be familiar with the surgical indications and the anatomy ofthe popliteal area.
