Chapter Ⅰ The clinical epidemiological study of diabetes mellitus complicated with gastroesophageal reflux diseaseObjective:To examine the incidence of gastro-oesophageal reflux disease (GERD) and its associated factors in patients with type2diabetes mellitus.Methods:In500DM outpatients, we conducted a QUEST inquiry and considered those showing a QUEST score of12or higher as having GERD. We surveyed clinical variables (Gender, age, smoking, alcohol drinking, BMI, duration of diabetes,fasting blood glucose,diabetes complications) to investigate their association with the onset of GERD.Results:1.The incidence of GERD in diabetic group(14.2%) was significantly higher than in non-diabetic group(7%)(P<0.05).2. The incident of GERD in the group over65years old was the hishest(16.9%),while in the group under45years old was the lowest(7.4%).3. The GERD incidence of the female in the diabetic group(14.8%) was higher than that in the control group(5.6%)(p<0.05).4.GERD was correlated with duration of diabetes, complications of diabetes but it was not found obviously correlated with smoking,alcohol drinking,BMI,fasting blood glucose.Conclusion:There is a high prevalence of GERD in diabetes mellitus patients; The incident in the group over65years old was higher than other groups; The incidence in the female was increasing when hyperglycemia occurs. GERD was correlated with duration of diabetes, complications of diabetes.Chapter Ⅱ The plasma levels and clinical significance of SP and VIP in the patients of diabetes complicated with GERD Objective:To investigate the levels and significance of substance P (SP) and vasoactive intestinal peptide (VIP) in the patients of diabetes complicated with GERD.Methods:Using ELISA method to determine the plasma SP and VIP levels in the control group (30cases), GERD group (30cases), diabetic group (30cases) and diabetes complicated with GERD group (30cases).Results:1. The plasma VIP levels in the Ⅰ、Ⅱ、Ⅲ group are significantly higher than the control group(P<0.01). The SP levels are significantly lower than one (P<0.05).2. The plasma VIP level in the Ⅲ group is significantly higher than that in the I and II group,The SP level is significantly lower than them (P<0.05).3. The plasma SP levels in the Ⅱ、Ⅲ groups are (64.2±8.33) ng/L,(48.4±9.74) ng/L (P<0.05) and they are negatively correlated with the duration of diabetes;The plasma VIP levels in the Ⅱ、Ⅲ groups are (67.7±13.3) ng/L,(83.1±18.5) ng/L(P <0.05) and they are positively correlated with the duration of diabetes.Conclusion:The decreasing of SP and the increasing of VIP play an important role in the development of diabetes complicated with GERD.
