| ObjectiveFetal congenital heart disease (CHD) is the most common disease of birth defects, isthe leading causes of infant deaths and disability, which has turned out to be a majorpublic health problem in influencing the children’s health and people’s quality of life.In the wake of social economy development, environmental pollution is aggravating, anincreasing number of environmental toxic elements occur in our lives and attractnumerous attentions to their harm to the fetus. This study explores the relationshipbetween chemical elements’ content in pregnant women’s and the fetal CHD to providescientific basis for primary prevention of CHD.MethodTo investigate61pairs case-control pregnant women during January,2010toDecember,2011who came from Fujian Provincial Maternity and Child Care Center, weadopted a hospital-based1:1matched case-control study. These pregnant women werematched according to the age and gestational age. All the objects were surveyed usingunification questionnaire and their hairs were collected as samples to test its arsenic,aluminum, selenium, antimony, lead, barium, beryllium, boron, magnesium, calcium,titanium, vanadium, chromium, manganese, iron, cobalt, nickel, copper, zinc, strontium,tin, tungsten, molybdenum, cadmium content by using inductively coupled plasma-massspectrometry (ICP-MS).Nonparametric test, univariate and multivariate Logisticregression analysis were adopted in this study by using SPSS19.0, a kind of statisticalanalysis software, to analyze the pregnant women hair chemical elements’ content and itsrelated factors of fetal CHD.Result1.The beryllium, arsenic, titanium, vanadium, aluminum, lead, antimony,barium,strontium, selenium contents in the hair from CHD group were significantly higher thanthose from control group, the nonparametric test results were statistically significant (P<0.050).2. The antimony, boron, magnesium, calcium, chromium, manganese, iron, cobalt, nickel,copper, zinc, molybdenum, cadmium, tin contents in the hair from the cases of pregnanthair were not significantly higher than those from control group, the nonparametric testresults were not statistically significant(P>0.050).3. By the single-factor Logistic regression, the study showed the risk factors of CHDwere the increases of arsenic, selenium, antimony, barium contents in the hair of pregnantwomen.4. By the single-factor Logistic regression, the study showed the risk factors of CHDwere living to urban-rural fringe, contacting noises, using air fresheners, cooking (at first3months in pregnancy), exposing to chemical preparation during pregnancy, the protectfactors may be the high level of eduction,high income.5. Arsenic contents (OR2=3.560,95%CI:0.385~32.898; OR3=11.696,95%CI:0.753~181.563;OR4=14.923,95%CI:0.976~228.189),selenium contents(OR2=1.306,95%CI:0.228~7.468;OR3=5.173,95%CI:0.964~27.758;OR4=54.499,95%CI:6.260~474.493),barium contens(OR2=9.701,95%CI:1.303~72.209;OR3=11.799,95%CI:1.704~81.686;OR4=11.266,95%CI:1.359~93.408),contact to noises duringpregnancy (OR=5.617,95%CI:0.854~36.936) and exposure to chemical preparationduring pregnancy(OR=5.862,95%CI:1.158~29.671)were the key risk factorscontributing to CHD,(OR=7.932,95%CI:1.098~57.271) were the key risk factorscontributing to CHD, while the high level of education (OR2=0.106,95%CI:0.018~0.644;OR3=0.097,95%CI:0.021~0.452) was the protective factor for CHD bymultivariate Logistic regression analysis.6. As the CHD group being devided by the complexity of CHD and the condition ofextracardiac anomalies, the arsenic, selenium, barium contents’ differences in variousCHD groups were not statistically significant (P>0.05).Conclusion1.An increase risk of CHD in pregnant women was observed in the study as their arsenic,selenium and barium contents increased, which showed the significant relationshipbetween arsenic, selenium and barium contents in pregnant women’s hair and the fetalCHD. 2.The titanium, vanadium, aluminum, lead, beryllium, strontium contents in the hair maybe associated with the occurrence of fetal CHD.3.The calcium, magnesium, boron, chromium, manganese, iron, cobalt, nickel, copper,zinc, tin, tungsten, molybdenum, cadmium contents in the hair was not associated withthe occurrence of fetal CHD.4. Contacting noises and exposing to chemical preparation during pregnancy might be therisk factors for CHD.The high level of education might be a protective factor for CHD.5. The arsenic, selenium and barium contents in pregnant women’s hair has nothing to dowith fetal cardiac anomalies’ complexity and the condition of extracardiac anomalyes. |
