| ObjectiveThrough the observation of Radix Astragali cassia twigs five effect of Decoction on serum DPN in patients with Hcy, CRP, MNCV, SNCV and other indicators of tibial nerve, to investigate the effect and mechanism of classical prescription Radix Astragali cassia twigs Five Decoction in the treatment of DPN, provide a new treatment method with reliable curative effect for prevention and treatment of DPN with traditional, enrich and develop the theory of Chinese Medicine. MethodsFrom 2013 January to2014 January in Anhui Provincial Hospital of Traditional Chinese medicine and the Bengbu Medical College Second Affiliated Hospital Outpatient and ward met the inclusion criteria of 60 DPN patients randomly were divided into the treatment group 30 cases and control group of 30 cases. 60 patients with routine diabetes diet and exercise guidance, reasonable control related indicators of blood glucose, blood pressure, lipids etc.according to the conditions of patients. Control group: oral Mecobalamin Tablets 0.5 mg/ time, 3 times a day. Treatment group: Radix Astragali cassia twigs Five Decoction, 1 day of 1 agentsdecoction, two course of treatment. Related indexes were detected before and after treatment.Fasting 12 hours, early in the morning to gather venous blood detection of 5ml, FPG, 2h PG, TC,TG oxidase method, method for the determination of Hb A1 c with automatic high pressure liquid chromatography, with enzymatic cycling assay for detection of Hcy, detection of CRP by immunoturbidimetry. EMG detection MNCV, SNCV tibial nerve, tibial nerve stimulation point ofpopliteal fossa and medial malleolus, the recording electrode was short abductor muscle of the toes. Clinical efficacy indexes by the Toronto clinical scoring system and TCM syndrome score,the correlation index contrast, summarizes the comprehensive curative effect. Results(1)After treatment, FPG, 2h PG, TC, TG decreased significantly(p<0.01), compared with the control group(p<0.05), the control group after treatment, FPG, 2h PG, TC, TG decreased,but no statistical significance, the treatment group, the control group after treatment Hb A1 c was significantly decreased(p<0.01), showed that the control nearly two months of the two groups of patients with good blood glucose.(2)The two groups after treatment of tibial nerve MNCV,SNCV compared with those before treatment were improved, the treatment group(p<0.01),control group(p<0.05) treatment group, improve the degree of tibial nerve MNCV, SNCV are better than that of control group(p<0.05).(3)Hcy, CRP two groups after treatment than before treatment were decreased(p<0.01 or p<0.05), compared with the control group, treatment group Hcy, CRP decreased significantly(p<0.01), there is statistical significance.(4)The two groups after treatment the Toronto score, TCM syndrome integral comparison of difference,there is statistical significance(p<0.01), the treatment group in improving the Toronto score and clinical symptoms are better than the control group(p<0.01).( 5) Two groups of treatment effect comparison :comprehensive treatment group total effectiveness 93.3%, significantly better than the control group 66.7%, there was significant difference(p<0.01). ConclusionThe clinical study showed that using Radix Astragali cassia twigs Five Decoction Combined with Mecobalamin Tablets treatment of DPN in improving the clinical symptoms,improve the motor and sensory nerve conduction velocity, reduce blood Hcy, CRP is superior tosingle Mecobalamin Tablets treatment, safe and reliable. The mechanism of action may be regulated by the integral of DPN patients, the clinical advantages of traditional Chinese medicinecompound play multi way, multi targets and multi system, thereby improving the symptoms andindexes of diabetic peripheral neuropathy. |
