Objective: Study the interleukin-18 and interleukin-6(IL-18, IL-6) gene polymorphism and the allele and genotype distribution in patients with type 2 diabetic nephropathy, analyze the relationship between their serum level,the genotype changes and type 2 diabetic nephropathy.Methods: using polymerase chain reaction-restriction fragment length polymorphism(PCR RFLP) method, adopts the method of case-control study,detect the genetic polymorphisms of IL-18 and IL-6 in 380 patients with type 2 diabetes(170 cases with diabetic nephropathy) and 190 normal people, including the IL-18-137-G/C gene promoter, IL-6 gene-634-C/G site promoter, using enzyme-linked immunosorbent assay(ELISA) method to detect the serum levels of IL-18 and IL-6, and detected the levels of triglyceride(TG), fasting plasma glucose(FPG), low density lipoprotein(LDL-c), apolipoprotein AI(apo AI), apolipoprotein B(apo B), high-density lipoprotein(HDL-C) and total cholesterol(TC).Results:1 IL-18 gene-137-C/G polymorphism loci GG genotype frequency and G allele frequency(56.5%, 73.2% respectively) in diabetic nephropathy group was lower than that in diabetic group(68.1%, 81.2% respectively) and normal control group(71.6%, 84.5% respectively), there were statistical differences(all P<0.05).2 Serum IL-18 and IL-6 level in the diabetic nephropathy group were significantly higher than that in diabetes without kidney disease group and the control group(P<0.01).3 The distribution of IL-18 gene-137-G/C gene polymorphism and IL-6 gene- 634-C/G polymorphism in the diabetic nephropathy group and normal healthy controls had significant difference(P<0.05), risk analysis of the allele frequency, risk of diabetic nephropathy in patients who carried IL-18 gene-137-G/C genotype C allele was 1.846 times higher than control group(OR = 1.846, 95% CI: 1.358 ~ 2.646), patients who carried IL-6 gene-634-G/C genotype C allele was 1.973 times higher than control group(OR = 1.973, 95% CI: 1.432 ~ 2.794),the serum IL-18 and IL-6 levels of patients who carried C allele was significantly higher than not carried it.Conclusion: Phenomenon of interleukin 18, 6 gene polymorphism is associated with the onset of diabetic nephropathy, its high expression level will increase the risk of diabetic nephropathy.
