Objective: this topic through the observation of conventional western medicine on chronic heart failure(CHF) treatment at the same time, to study group on the basis of routine western medicine combined with compound Zhenwu granules, to affect the observation compound Zhenwu granules on the serum NT-pro BNP, LVEF and CHF in patients with clinical symptoms of TCM, and to evaluate the effect of compound Zhenwu granules in the treatment of CHF in the analysis, and to explore its possible mechanism.Method:1. according to CHF diagnostic criteria of TCM syndrome, heart function of NYHA standards, grading diagnosis standard, into a unified formulation of standards. 80 cases of CHF patients were included, with reference to the standard were randomly divided into study group 40 cases and control group 40 cases.2. the control group of 40 patients treated with conventional western medicine treatment, the study group of 40 patients with compound Zhenwu granules on the basis of the above, the course of treatment was 6 weeks.3.Compared before and after the treatment effect of TCM syndromes, symptoms and quality of life of integral counting; heart function(NYHA), heart rate(average 24 h dynamic ECG) and ultrasonic Beckoning figure LVEF comparison; comparison of serum NT-pro BNP, MMP-3 concentration detection.4. All measurement data is "x—± s" represented by paired sample test was used for comparison within the group, using independent sample t test to compare between the two groups, count data using χ2 test, P <0.05 was considered statistically significant, P <0.01 was the difference was statistically significant. Using SPSS17.0 statistical software for statistical analysis of all the data.- Traditional users.Results:1. Comparison between before and after treatment in the study group: to improve the TCM syndrome after treatment, clinical symptoms and quality of life were lowercompared with the previous points; NYHA functional class, echocardiographic parameters were improved compared with the previous average heart rate; serum NTpro BNP, MMP 3-concentrations were decreased compared with the previous, and before significant treatment difference was statistically significant(P <0.01).2. The control group before and after the treatment in contrast: to improve the therapeutic efficacy of TCM syndromes after treatment, symptoms and quality of life of TCM were decreased than before; NYHA cardiac functional grading, ultrasonic Heartbeat figure index were improved, compared with the previous average heart rate slows down; the serum NT-pro BNP, MMP-3 concentrations were decreased than before, with statistical significance compared with that before treatment the difference(P<0.01).3. Comparison between the two groups after treatment: study group after treatment syndromes effective than the control group(P <0.05); the study group after treatment clinical symptoms and decreased quality of life and the integral effect than the control group(P <0.01); after the study group therapy NYHA class, echocardiographic parameters were improved and the results better than the control group(P <0.01); the study group after treatment reduced the average heart rate more significantly(P <0.01); study group after treatment serum NT-pro BNP, MMP 3-concentration decreased more than the control group decreased significantly(P <0.01); two groups were not serious adverse reactions. Conclusion:Compound Zhenwu Granules Combined with conventional western medicine in the treatment of CHF curative effect is better than pure western medicine, can significantly improve the life quality of patients, the mechanism may be: 1 inhibited activation of RAAS system, reduce the sodium and water retention, reduce the load on the heart, improve heart function of patients; accumulation of 2 inhibition of extracellular matrix degradation and collagen, adjust the balance of MMPs/ TIMPssystem, delayed myocardial remodeling in CHF patients; 3 inhibition of sympathetic activation, enhanced the activity of the vagus nerve, reduce myocardial oxygen consumption, and thus improve the cardiac function. |
