ObjectiveThe aim of this study is to explore the role and clinical significance of B7-H4proteinand HK6protein in the pathogenesis and progression of ovarian neoplasm by detectingtheir expression in tissue of normal ovary,epithelial ovarian benign tumors, borderlinetumor and malignant tumor.MethodsUsing ovarian neoplasm as the research object, normal ovarian as the control group,examined the expression of B7-H4protein and HK6protein in10cases with normalovarian tissue,15cases with benign ovarian neoplasms,10cases with borderline ovarianneoplasms and45cases with malignant ovarian neoplasms by immunohistochemicalstaining and compared them and statistically analyze whether the expression of B7-H4protein and HK6protein correlated with the clinicopathologic variables (the patients,age,the FIGO clinical pathologic stage, WHO pathologic grade, lymph node metastasis, et al)in patients with malignant ovarian neoplasms.Results1.The positive expression rate of B7-H4protein in malignant ovarian neoplasmstissues was80.0%(36/45), that in the borderline ovarian neoplasm tissues was30.0%(3/10),in the benign ovarian neoplasm tissues was20.0%(3/15), in the normal ovarian tissue was0.0%(0/10). The positive expression rate in malignant ovarian neoplasms tissues wassignificantly higher than that in the latter three groups and the difference was statisticallysignificant(χ2=31.192, P=0.000),but the difference between the benign (20.0%) andborderline (30.0%) ovarian neoplasm tissues was not statistically significant(χ2=0.009,P=0.924).2. The positive expression rate of B7-H4protein in malignant ovarian neoplasmstissues:(1) That in the patients whose ages are below50was75.0%(6/8) and in the patients whose ages are above50was81.1%(30/37), the difference between them was notstatistically significant (χ2=0.000, P=1.000).(2) That in the ovarian serous carcinoma was96.2%(25/26), in the ovarianendometrioid carcinoma was83.8%(5/6), in the ovarian clear cell carcinoma was66.7%(4/6) and in the ovarian mucinous carcinoma was28.6%(2/7). The positive expression ratein the ovarian mucinous carcinoma was significantly lower than that in the former threegroups and the difference was statistically significant (χ2=10.161, P=0.001). The differencebetween the former three groups was not statistically significant (χ2=4.052, P=0.132).(3) B7-H4protein in late-stage (stage III or Ⅳ) ovarian carcinoma tissues was morefrequently expressed than that in early-stage (stage I or II), the positive expression ratewere96.5%(27/28) vs52.9%(9/17), and the difference was statistically significant(χ2=9.932, P=0.002).(4) The positive expression rate of B7-H4protein in low-grade (grade2or3) ovariancarcinoma tissues was89.2%(33/37),was higher than that in the high-grade (grade1)group37.5%(3/8), the difference was statistically significant (χ2=10.984, P=0.001).(5) The positive expression rate is significantly higher in ovarian carcinomas withlymph node metastases than those without lymph node metastases, the positive expressionrate were94.7%(18/19) vs69.2%(18/26), the difference was statistically significant(χ2=4.464, P=0.035).(6) The positive expression rate of B7-H4protein in ovarian carcinomas with asciteswas85.7%(12/14), that in ovarian carcinomas without ascites was77.4%(24/31), thedifference between them was not statistically significant (χ2=0.058,P=0.809).3. The positive expression rate of HK6protein in the normal ovarian tissue was10%(1/10), in the benign ovarian neoplasm tissues was13.3%(2/15), in the borderline ovarianneoplasm tissues was50.0%(5/10) and in the malignant ovarian neoplasms tissues was60%(27/45). The positive expression rate in the benign ovarian neoplasms tissues wassignificantly lower than that in the borderline and the malignant ovarian neoplasms tissuesand the difference was statistically significant (χ2=9.490, P=0.002). But the differencebetween the borderline ovarian neoplasm tissues(50.0%) and the malignant(60.0%) ovarianneoplasm tissues was not statistically significant (χ2=0.051, P=0.822).4. The positive expression rate of HK6protein in malignant ovarian neoplasmstissues:(1) That in the patients whose ages are below50was62.5%(5/8), in the patientswhose ages are above50was59.5%(22/37), the difference between them was not statistically significant (χ2=0.000, P=1.000).(2) The positive expression rate of HK6protein in the ovarian serous carcinoma was65.4%(17/26), in the ovarian mucinous carcinoma was57.1%(4/7), in the ovarianendometrioid carcinoma was50.0%(3/6) and in the ovarian clear cell carcinoma was50.0%(3/6), the difference between every two groups was not statisticallysignificant(χ2=0.833,P=0.842).(3) HK6expressed in late-stage (stage III or Ⅳ) ovarian carcinoma tissues morefrequently than that in early-stage (stage I or II) ovarian carcinoma tissues, the positiveexpression rate were75.0%(21/28) vs35.3%(6/17), the difference was statisticallysignificant (χ2=6.949,P=0.008).(4) The positive expression rate of HK6protein in low-grade (grade2or3) ovariancancer tissues was higher than that in the high-grade (grade1) group, the positiveexpression rate were70.3%(26/37) vs12.5%(l/8) and the difference was statisticallysignificant (χ2=6.898, P=0.009).(5) The positive expression rate is significantly higher in ovarian carcinomas withlymph node metastases than those without lymph node metastases, the positive rate were84.2%(16/19) vs42.3%(11/26), the difference was statistically significant (χ2=8.031,P=0.005).(6)The positive expression rate of HK6protein in ovarian carcinomas with ascites was64.3%(9/14), that in ovarian carcinomas without ascites was58.2%(18/31), the differencebetween them was not statistically significant (χ2=0.004,P=0.948).5. There was a weakly linear positive correlation between the expression of B7-H4protein and HK6protein(r=0.218, P=0.039).Conclusion1. B7-H4protein and HK6protein express highly in the ovarian neoplasm tissues andthe expression rate is elevating with the progression of the clinical pathologic stage. Thatsuggests that B7-H4protein and HK6protein may participate in the progression of ovarianneoplasm.2. The expression of B7-H4protein and HK6protein in the malignant ovarianneoplasm tissues is correlated with the FIGO clinical pathologic stage, the WHOpathologic grade, lymph node metastasis and is not correlated with ascites and the patients,age. The expression of B7-H4protein is also correlated with the histological type, but theexpression of HK6protein is not.3. There was a weakly linear positive correlation between the expression of B7-H4 protein and HK6protein, that suggests that B7-H4protein and HK6protein may play asynergic role in the pathogenesis and progression of ovarian neoplasm.
