Objective: To investigate the diagnostic value and practical application ofreal-time transrectal ultrasound elasticity imaging in prostate biopsy, and tochoose a reasonable method for prostate biopsy based on a certain circumstance.Method: To underwent real-time transrectal ultrasound elastographyexamination and prostate biopsy on41patients (average age is71.32years, agefrom52to82years) who have PSA increased(PSA between8and100ng/ml) inthe duration between Aug.2014and Feb.2015, to perform6-pin,8-pin,10-pinpuncture according to the result from real-time transrectal ultrasoundelastography, and to compare the captured image and puncture spot dependingon the pathology result.Results:41patients undertook first trial of transrectal ultrasound prostatebiopsy, it has diagnosed26cases (accounting for68.5) with prostate cancer,15patients with benign prostatic hyperplasia, and there are no patient with severecomplications after puncture. For the case of Benign prostatic hyperplasia, theelastic stiffness attains higher number of pins within1-3points, mainlyconcentrated in2points, it shows in the color between green and blue inreal-time ultrasound elastography, which it mainly on green. In prostate cancer,among the elastic stiffness assessed4-5points, it shows in the color betweengreen and blue in real-time ultrasound elastography, which it mainly on blue orthe tissue around it shows in blue. Less than or equal to3points in elasticstiffness assessment has greater possibility in benign prostatic hyperplasia,prostate cancer is more likely in4-5points. With the Gleason grade of6points,7points,8points,9points,10points, the corresponding elastic stiffness grade significantly increase, the higher the Gleason leveled, the higher elasticityhardness grades. Its elastic hardness grading has a certain significance inpathological grading of prostate cancer. Comparing between the length andelasticity hardness grade of the cancer accounting in puncture tissue biopsytissue, the performance of them of them is not obvious, and it does not havestatistical significance in its distribution in the statistics calculation. Theelasticity hardness grading in Benign prostatic hyperplasia and prostate cancer isthat P=0.001, while Gleason grading is7points where P=0.002,8points whereP=0.001,9points where P=0.001, and the size of prostate cancer is thatP=0.054.Conclusion:Real-time elasticity elastography and hardness grading isactually useful in the diagnosis of prostate disease, the greater the hardnessgrade leveled, the higher the possibility of prostate cancer. Elasticity hardnessgrading is positively correlated with Gleason grading. Prostate cancer has highergrade of malignancy with high grade of elasticity hardness. The size of prostatecancer has no obvious correlation with elasticity hardness grading.
