Objective:Adverse reactions of food were caused by food ingredients and food additives in the symptoms and side effects. Adverse reactions of food include food allergy and food intolerance.Food allergy was mediated by IgE at belongs to anaphylaxis.Food intolerance was mediated by IgG,it belongs to the delayed hypersensitivity.To investigate the allergization to allergens specific IgE antibody and IgG antibody in serum of patients with diseases of digestive system(gastroesophageal reflux disease,peptic ulcer disease.irritable bowel syndrome,intestinal polyps) and cases of normal control group.In order to provide a diagnostic bases for clinical diagnosis and treatment of the digestive system diseases which adverse reactions related to food.Methods:From March 2013 to April 2015,140 patients with gastrointestinal diseases were selected as experimental group in the first affiliated hospital of Dalian Medical University.40 patients with Gastroesophageal reflux disease were detected.34 patients with peptic ulcer disease were detected.33 patients with irritable bowel syndrome were detected.33 patients with intestinal polyps were detected.EUROLINE method was applied to detect the serum concentration of allergen specific IgE antibody from the patients and 100 cases of normal control group.ELIS A method was applied to detect the serum concentration of allergen specific IgG antibody from the 140 patients and 20 cases of normal control group. According to the different type of diseases to observe and compare the serum test results.Results:1.These allergens whose serum allergen specific IgE antibody were positive results included marine fish combination,fresh water fish combination,shrimp,crabs,hen egg protein,peanuts,soybeans,milk,tree combination,humulus scandens,prevalent ragweed,mugwort,dust mite combination,house dust,mould combination,blattodea and dog epithelium.These allergens whose serum allergen specific IgE antibody were negative results included beef,mutton and cat hair.The group of four kinds of diseases whose serum allergen specific IgE were deteced,one kind of allergens was 19.29%, two kinds of allergens was 6.43%.The higher positive rates of food allergens were marine fish combination(15.71%),crab(12.14%) and fresh water fish combination(5.71%),food allergen in the cases were given priority to with 1-2 levels of allergies, it was 32.14% of the food allergen in the total.The higher positive rates of inhaled allergens were dust mite combination(12.14%) mugwort(5.00%),blattodea (4.29%) and tree combination(4.29%),inhaled allergens was given priority to with 1-2 levels of allergies, inhaled allergens were 27.86% of the cases.2.The positive rate of the serum specific IgE from the patients was 36.43%.The positive rate of the serum specific IgE from the healthy persons was 13.00%.There was statistically significant difference between the patients and the controls(P<0.05).The positive rate of the serum specific IgE from the patients with GERD was 35.00%,the positive rate of the serum specific IgE from the patients with irritable bowel syndrome was 33.33%,the positive rate of the serum specific IgE from the patients with intestinal polyps was 57.58%,there were all significant difference between these gastrointestinal disease and the controls(P<0.05).The positive rate of the serum specific IgE from the patients with peptic ulcer disease was 20.59%,There was no significant difference between peptic ulcer disease and normal control group (P> 0.05).3.In the patients whose serum specific IgG antibody were positive results,these allergens whose serum allergen specific IgG antibody were positive results included egg,milk,shrimp,crab,tomato,wheat,corn,mushroom and pork.These allergens whose serum allergen specific IgG antibody were negative results included rice,beef,chicken,soybean and cod.The group of four kinds of diseases whose serum allergen specific IgG were deteced,one kind of allergens was 41.43%, two kinds of allergens was 14.29%.The higher positive rates of allergens were egg(42.86%),mushroom(11.43%) and milk(9.29%).Allergens were given priority to with 1-2 levels of allergies,it was 53.28% of the total.4.The positive rate of the serum specific IgG from the patients was 61.43%.The positive rate of the serum specific IgG from the healthy persons was 30.00%.There was statistically significant difference between the patients and the control s(P<0.05).The positive rate of the serum specific IgG from the patients with GERD was 50.00%.The positive rate of the serum specific IgG from the patients with peptic ulcer disease was 55.88%.The positive rate of the serum specific IgG from the patients with irritable bowel syndrome was 69.70%.The positive rate of the serum specific IgG from the patients with intestinal polyps was 72.73%.There were significant difference between patients with irritable bowel syndrome、intestinal polyp and normal control group(P<0.05).5.The patients whose serum specific IgG and IgE were both detected. The specific IgG and IgE antibody were both positive accounted for 25.00%,The specific IgG and IgE antibody were both negative performance accounted for 26.43%.Kappa test was used in the consisency of the two kinds of test methods, Kappa<0.4.It was conclusioned that the two method were inconsitent.lt was said that joint detection is more beneficial to the diagnosis.Conclusion:The positive rate of the serum specific IgE from the patients with diseases of GER.D、irritable bowel sydrome, intestinal polyps were higer and significant difference than the controls.The food allergens of patients in digestive department were given priority to fishes and crab.The inhaled allergens of patients in digestive department were given priority to dust mites and blattodea.The positive rate of the serum specific IgG from the patients with diseases of irritable bowel sydrome、intestinal polyps were higer than the controls. There were significant difference between patients and normal control group.In the patients with positive specific IgG,the higher positive rates of allergens was egg.The result was conclusioned that the two method were inconsitent in the positive rate.Through the analysis of food allergy and intolerance in the digestive system disease,we find that determination of food allergen IgE and IgG in serum at the same time would important to promptly detecting the particular food allergizable to patients with diseases of digestive system and further to patients diet guidance.
