ABSTRACT Objective:To analyze the clinical pathologic features of prostate adenocarcinoma patients50years old or younger.Methods:Between January2008and April2014, we studied21cases of prostate cancer in men aged under50years (age:42-49yrs). The prostate specific antigen (PSA) at diagnosis was>4ng/ml in all patients and averaged38.39ng/ml.20patients were diagnosed by prostate biopsy, the other was diagnosed after TURP operation.3(14.3%) cases had a Gleason score of6,6(28.6%) cases had a Gleason score of7,7(33%) cases had a Gleason score of8,5(23.8%) cases had a Gleason score of9. Pathologic stage was T2in19cases, T3in1case,T4in1case, metastasis was found in3cases.5patients received hormonal therapy,16patients underwent radical prostatectomy. Results:During the follow-up,14of the16patients who underwent radical prostatectomy remains free of recurrence,1patient received external beam radiotherapy after one month of radical prostatectomy because tumor extended through the prostatic capsule,1patient received external beam radiotherapy after18months of radical prostatectomy because of biochemical recurrence. In5patients who had hormonal therapy,1patient had castration resistant prostate cancer after24months of hormonal therapy.2patients died at63and65months respectively,1patient lost to follow-up.Conclusion:Men under50years diagnosed with prostate cancer who underwent radical prostatectomy tend to have favorable outcomes. Among men with advanced prostate cancer, the younger men had a particularly poor prognosis.
