Objective: In order to formulate a scientific and reasonable intervention measures for coal miners and improve the quality of life of hypertension patients. This experiment use a factorial design method to observed the influences of 8 weeks of walking and health education on various indicators of patients with different levels of blood pressure. The indicators include blood pressure, blood glucose, blood lipid and BMI, waist hip ratio, quality of life, etc.Methods: Selected 134 cases of hypertension patients woking at The Central Factory of Datong Coal Mine Group.Then randomly divided them into four groups using factorial design method. Walking group, health education group, walking+health education group and control group were included.Four groups were wearing pedometer.Walking group and walking+ health education group wearing the heart rate watch for monitor and control heart rate and exercise intensity.The level of exercise intensity is 70% HRmax.Health education intervention takes the form of lectures and the relevant handbook.Before and after the intervention, blood pressure, blood glucose, blood lipid, BMI, WHR, quality of life index and so on were measured.To observed the influences of 8 weeks of walking and health education on various indicators and analyzed the interactive effect.Results:(1). Exercise adherence: During the intervention There are 32 people take part in the experiment in walking+health education group and five people quit, the lost rate was 15.62%.There are 31 people take part in the experiment in walking group and thirteen people quit, the lost rate was 41.93%.There were significant difference between two groups(P=0.021).The step frequency of walking+ health education group is 36.78 times, more than walking groups which is 32.28 times(P=0.034). The average step number of walking+health education group is 13099.But walking group is 10345.There were significant difference between two groups(P=0.006).To sum up, walking+health education can effectively enhance the walking compliance of the hypertension patients.(2).Blood pressure: The blood pressure before and after the intervention were tested and the results showed that:walking can effectively reduce systolic pressure and diastolic pressure of hypertension patients.In walking+health education group, systolic pressure reduce by 6.74 mm Hg, diastolic pressure reduce by 9.37 mm Hg.In walking group, systolic pressure reduce by 3.50 mm Hg, diastolic pressure reduce by 4.72 mm Hg. In health education group, systolic pressure reduce by 2.09 mm Hg, diastolic pressure increase by 1.45; In control group,systolic pressure reduce by 1.60 mm Hg, diastolic pressure reduce by 2.00. The intervention factorial design of covariance analysis showed that walking and health education have no interaction effect to lower blood pressure(systolic pressure P = 0.668, diastolic pressure P = 0.397). Both systolic and diastolic blood pressure, walking effects were statistically significant(systolic pressure P = 0.003, diastolic pressure P < 0.001); And the effect of health education intervention on blood pressure has no statistical significance(systolic pressure P = 0.411, diastolic pressure P = 0.361).(3).Glucose and Lipid: 8 weeks of walking can reduce the blood glucose of hypertension patients.Walking group reduce by 0.392 mmol/L,walking+health education group reduce by 0.688 mmol/L,P-value was 0.04.In blood lipid,Walking has no effect on TG, TC, HDL, LDL and other blood lipid parameters. In addition, health education had no effect on the index(P> 0.05), and Walking and health education had no interaction effect on blood glucose and blood lipid(P> 0.05).(4).Form indicators: There is no interaction effect between walking and health education intervention.8 weeks of walking has certain effect on lose weight.Walking group reduce by 0.667 kg,walking+health education group reduce by 0.100 kg,( P =0.033).Besides,walking can reduce hip circumference, waist circumference and BMI. And the intervention effect of health education only on the waist circumference was statistical significance.(5).Quality of life: The factorial design analysis of variance showed that before and after the intervention, the effect of walking interventions and health education in all areas of quality of life were not statistically significant.Conclusions:(1).Moderate-intensity exercise(70% HRmax) can reduce systolic and diastolic pressure of hypertension patients, and can regulate blood glucose and reduce weight, BMI, hip circumference, blood sugar and other indicators.So,walking has a certain effect on prevention of obesity and diabetes.(2).Health education simplely has no effect on blood pressure,blood glucose,blood lipids,body mass index,etc.(3).Health education can improve exercise adherence in hypertensive patients.(4).There is no interaction effect between walking and health education on the various indicators.
